摘要
【目的】观察补肾健脾化瘀中药对围绝经期妇女性激素及泌尿系统感染的影响。【方法】选择围绝经期患者377例,随机分为中药组(188例)及西药组(189例)。中药组给予口服补肾壮骨冲剂治疗,西药组应用倍美力合安宫黄体酮治疗。主要观察治疗前后性激素[雌二醇(E2)、卵泡刺激激素(FSH)、促黄体生成素(LH)]及泌尿系感染发生率的变化。【结果】①脱失情况比较:中药组治疗2年后脱失53例,西药组脱失96例,两组比较差异有显著性意义(P<0.01),说明中药组患者的依从性较好。②性激素水平比较:与治疗前相比,治疗后两组均能明显升高E2水平,降低FSH、LH水平(P<0.05),且治疗后2年这种变化趋势较治疗后1年更为显著(P<0.05);治疗后组间比较差异有显著性意义(P<0.05),说明中药组在改善性激素水平方面尚未达到西药组水平。③泌尿系统感染发生率比较:中药组为6.7%,西药组为6.5%,组间比较差异无显著性意义(P>0.05),说明中药组患者的泌尿系统感染率与西药组相仿。【结论】补肾健脾化瘀中药治疗围绝经期妇女,能纠正紊乱的性激素E2、FSH及LH水平,降低泌尿系统感染率,且具有无副作用、经济、患者依从性好等优点。
Objective To investigate the effect of herbs of tonifying kidney, strengthening spleen and removing blood-stasis on sexual hormones levels and urinary tract infection in perimenopausal women. Methods Totally 377 perimenopausal outpatients admitted in Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA from March, 2002 to March, 2006 were enrolled into the investigation. The outpatients were randomized into two groups： the herbal medicine group （N = 188） received Bushen Zhuanggu Granules, and the western medicine group （N = 189） received oral use of Conjugated Estrogens Tablets and Medroxyprogesterone Acetate Tablets. The changes of serum sexual hormone levels such as estradiol （ E2 ） , follicle-stimulating hormone （FSH） and luteinizing hormone （LH） as well as incidence of urinary infection were observed before and after treatment. Results After treatment for 2 years, dropout occurred in 53 patients of herbal medicine group and in 96 of western medicine group, and the difference was significant （ P 〈 0. 01 ） , indicating good compliance of these patients. Compared with those before treatment, the serum E2 level was increased, and FSH and LH levels were decreased in the two groups （P 〈 0.05）, the above changes being obvious as compared those after treatment for one year （P 〈 0.05）. The differences of sexual hormone levels between the two groups after treatment were significant （P 〈0.05）. The incidence of urinary infection was 6.7% in herbal medicine group and 6.5% in western medicine group （P 〉0. 05 ）. Conclusion Herbs of tonifying kidney, strengthening spleen and removing blood-stasis have an effect on regulating serum sexual hormones of E2, FSH and LH levels, and decreasing incidence of urinary infection in treating perimenopausal women, having no side effect.
