摘要
应用二维多普勒超声心动图对临床无心血管并发症的非胰岛素依赖型糖尿病(NIDDM)左室结构及功能进行检测,与正常组对照.结果:无并发症的NIDDM、IVST、LVM、LVMI与对照组呈非常显著性差异,左室收缩功能EF、Fs降低但无显著性差异,左室舒张功能FVI、1/3TFF、1/2TFF明显降低,有非常显著性差异.提示:无合并症的NIDDM心肌结构和功能的改变,可视为糖尿病性心肌病,本病的心肌肥厚及心肌功能的改变,与该病的合并症无关,是由本病的病理学基础所决定.指出:糖尿病性心肌病的主要特征是心肌结构和功能的改变,而LVM、LVMI、EF、Fs、FVL、1/3TFF、1/2TFF是评价本病的敏感而可靠指标.二维多普勒超声心动图应做为本病的首选诊断方法.
Left ventricular structure and function of noninsulin-dependent diabetes mellitus (NID-DM) patients without cardiovascular complications were studied by 2 - DE Doppler echocardiograpby. The control group was formed of normal subjects. Result; IVST. LVM, LVMI had very significant difference between NIDDM patients without cardiovascular complications and control group. Though EF, Fs of left ventricular contractile function decreased, there was no significant difference betweentwo group. FVI, 3/1TFF, 2/1TFF of left ventricular diastolic function had decreased markedly and had very significant difference between two groups. The result showed: changes of myocardial structure and function of NIDDM patients without complications can be regarded as diabetes cardiomyopathy. Myocardial hypertrophy and changes of myocardial function of the disease had no truck with complications of NIDDM and was being determined by pathological basic of this disease. Conclusion: the main characteristics of diabetes cardiomyopathy are changes of myocardial structure and function. LVM, LVMI, EF, Fs, FVI, 3/1TFF, 2/1TFF are sensitive and credible indexes that evaluate the disease. 2-DE Doppler echocardiography should be regarded as the first diagnosis method of the disease.
