摘要
目的探讨阴囊中隔皮瓣尿道成形术治疗长段的尿道狭窄的临床疗效。方法选择男性前尿道狭窄患者10例。年龄16～65岁,平均37岁,尿道狭窄长度2.0～5.5cm,平均3.0cm。手术彻底切除狭窄段尿道,取合适大小的带蒂阴囊中隔皮瓣,5～0可吸收肠线缝合成管状,与尿道端端斜面吻合,保留尿道支架管3～4周。结果10例手术均成功,手术时间150～230min,平均180min,随访1～35个月,平均16个月。阴茎外观满意,2例手术后1～2个月出现排尿变细,经扩张后排尿通畅。结论阴囊中隔尿道成形术治疗长段前尿道狭窄简单有效,并发症少,可作为长段前尿道狭窄临床治疗选择之一。
Objective To evaluate clinical therapeutic efficacy of scrotal dartos flap urethroplasty with vascular pedicle on anterior urethral stricture. Methods 10 male patients with average age of 37 （ 16 - 65 ） years having anterior urethral stricture were included. The length of uretharal siricture was 2.0 - 5.5 cm, average length was 3.0 cm. Narrowed urinary was cut completely by operation, feasible pedical skin flap of scorotal septum was cut, and sutured into tubular structure by adsorhabal catgut of 5-0, then was anastomosised with urethra inclined plane end to end. Silicone catheter was inserted into the reconstructed urethra for 3 ~ 4 weeks. Reasults All 10 cases had successful surgery,the operative time were about 150-230 rain,mean time is 180 rain. The following up time is about 1-35 monthes,the mean time is 16 monthes. Satisfied with the appearance of the penis,2 cases has weaked urinary stream about one to two monthes later, and recovered after urethra dilation. Conclusion scorotal septum urethroplasty is a simple method to treat anterior urethrostenosis with less complications, which can he considered as one of the therapies in clinic.
