输尿管镜下钬激光碎石术治疗上尿路结石梗阻致急性肾功能衰竭的体会

摘要 目的探讨输尿管镜下钬激光碎石术治疗上尿路结石梗阻致急性肾功能衰竭的疗效。方法2005年4月至2007年8月对25例上尿路结石梗阻致急性肾功能衰竭(血肌酐大于500μmol/L)患者行输尿管镜检查并行钬激光碎石术。结果25例患者肌酐恢复正常或接近正常,18例输尿管结石一次取净,5例经体外冲击波碎石后结石排出,2例碎石后肾盂残留小结石经口服排石药自行排出。结论对上尿路结石梗阻致急性肾功能衰竭患者,输尿管镜检查辅以钬激光碎石创伤小、疗效好,并可同时处理双侧输尿管结石。 Objective To evaluate the curative effect of ureteroscopic Ho Laser lithotripsy in the treatment of acute renal failure induced by upper urinary tract obstruction due to calculus. Methods There were 25 cases of acute renal failure ( serum creatine 〉 500 μmol/L) induced by upper urinary obstruction due to calculus, urgently treated by Ho Laser lithotripsy under ureteroscopy, from April 2005 to August 2007 in this hospital after stone fragmentation and removal, arid indwelling double - J stent was placed into the ureter at the same time. Results Serum creatine were decreased to normal levels after operation in all the 25 cases. The ureteral calculi were thoroughly removed on one session in 18 cases, 5 stones were removed after extracorporeal shock wave lithotripsy, and few residual stones found in the renal pelvis in 2 cases were spontaneously expelled after oral medication. Conclusion Ureteroscopic Ho Laser lithotripsy in the treatment of acute renal failure induced by upper urinary tract obstruction due to calculus is an effective treatment with short operative time, safety and minimal trauma. By using this procedure, both sides of ureteral stones can be removed on one session.
