摘要
目的:研究结缔组织生长因子(CTGF)在特发性肺纤维化(IPF)患者肺组织的表达及意义。方法:IPF组的肺组织标本来自35例IPF患者,对照组的正常肺组织标本来自30例肺叶切除术。用原位杂交结合免疫组化方法检测CTGFmRNA及其蛋白的表达,用免疫组化方法检测纤维连接蛋白(Fn)的表达。结果:IPF组CTGFm-RNA及其蛋白以及Fn的阳性面积百分比均高于对照组(P均<0.001);IPF组CTGFmRNA及其蛋白在增生的Ⅱ型肺泡上皮细胞、肺泡巨嗜细胞和肺间质成纤维细胞中的表达强度均高于对照组(P均<0.001),在细支气管上皮细胞中的表达无差异(P>0.05);IPF组CTGFmRNA及其蛋白的阳性面积百分比与Fn阳性面积百分比之间呈正相关。结论:IPF患者肺组织CTGFmRNA及其蛋白表达增高,并主要定位于Ⅱ型肺泡上皮细胞、肺泡巨嗜细胞和间质成纤维细胞,CTGF可能通过促进Fn的合成而在IPF中发挥作用。
Objective:To investigate the expression and significances of connective tissue growth factor in fibrotic lung tissue of patients with idiopathic pulmonary fibrosis(IPF).Methods:Fibrotic lung tissue specimens were obtained from a total of 35 patients with IPF by transbronchial lung biopsy(TBLB),30 normal lung specimens obtained from resection of thoracic surgery were used as controls.To investigate the expression of CTGF messenger ribonuclear acid(CTGF mRNA) and its protein using in situ hybridization and immunohistochemical staining respectively through comparing distribution grade in various cells(score) and positive proportion percentage.To investigate fibronection(Fn) expression using immunohistochemical staining through comparing positive proportion percentage.The correlation with the positive proportion percentage of CTGF mRNA and its protein and that of Fn were further analyzed.Results:(1)The positive proportion percentage of CTGF mRNA and its protein and Fn were significantly elevated in fibrotic lung tissue in patients with IPF compared to nomal lung tissue(P〈0.001);(2)CTGF mRNA and its protein were confined predominantly to proliferating type Ⅱ alveolar epithelial cells,fibroblasts and alveolar macrophages in fibrotic lung tissue in patients with IPF(P〈0.001).The distribution of them in tiny bronchial epithelial cells were found have no difference in two groups(P〉0.05).(3)The rank correlation coefficient of positive proportion percentage of CTGF mRNA and its protein with that of Fn was 0.51 and 0.64 respectively(P〈0.01) in fibrotic lung tissue of patients with IPF.Namely the expression of CTGF mRNA and its protein were found to have a close positive correlation with that of Fn in fibrotic lung tissue of patients with IPF.Conclusions:In the present study,we have shown that CTGF mRNA and its protein were expressed at a high level in fibrotic lung tissue of patients with IPF.The immunolocalization of CTGF was confined predominantly to prolife
