摘要
目的:评价氢质子磁共振波谱(1HMRS)对颅内常见肿瘤的诊断和鉴别诊断价值。方法:对经手术和病理证实的38例脑肿瘤(星形细胞瘤20例,脑膜瘤10例,转移瘤8例)患者行常规MRI和1HMRS扫描,采用软件包FuncTool进行分析,分别计算出肿瘤增强区和对侧正常区的N-乙酰天门冬氨酸(NAA)/胆碱(Cho)、NAA/肌酐(Cr)和Cho/Cr比值。结果:转移瘤、脑膜瘤均有NAA峰,峰值明显低于星形细胞瘤,转移瘤及脑膜瘤NAA/Cho和NAA/Cr比值与星形细胞瘤比较差异有统计学意义(P<0.05);转移瘤与脑膜瘤Cr峰较低或缺乏,星形细胞瘤Cr峰明显下降与正常各占50%;脑膜瘤Cho/Cr显著升高;星形细胞瘤Ⅰ~Ⅱ级与Ⅲ级~Ⅳ级的NAA/Cho、NAA/Cr和Cho/Cr比值差异有统计学意义(P<0.05)。结论:1HMRS可无创地分析脑肿瘤的代谢状况,对脑肿瘤的诊断、鉴别诊断均有较大的价值。
Objective: To study the value of proton MR spectroscopy (1HMRS) in the diagnosis and differential diagnosis of common brain tumors. Methods:Routine MR and proton MR spectroscopy were performed in 20 cases of astrocytoma, 10 cases of meningioma and 8 cases of metastases, who were proven surgically, pathologically and clinically. N-acetylaspartate/Choline (NAA/Cho), NAA/creatine (Cr) and Cho/Cr ratios were calculated and analyzed by Functool software. Results: Metastases and meningiomashowed lower NAA. NAA/Cho and NAA/Cr of meningioma and metastases were significantly lower (P 〈 0. 05) than astrocytomas. Cr peak of meningioma and metastases decreased or disappeared. Cho/Cr of meningioma was significantly higher than those of other tumors. Significant difference between low-grade (gradeⅠ-Ⅱ) and high-grade (gradeⅢ-Ⅳ) astrocytomas was found in NAA/Cho, NAA/Cr and Cho/Cr ratios(P 〈 0.05). Conclusion:l HMRS can be used as a supplementary non-traumatic method to analyze the metabolism of brain tumors and is very helpful in their diagnosis and differential diagnosis.
