摘要
目的:讨论X线、CT在诊断脊柱爆裂骨折中的价值。方法:回顾性分析120例腰椎爆裂骨折的影像学表现和临床资料。结果: X线、CT显示椎体自然形态不同程度消失,骨质密度低,并见碎骨片向椎体周围膨出。附件骨折(椎弓根间距增宽或变窄)压迫椎管,继发椎管狭窄,小关节紊乱,脱位、间盘突出,脊髓受压。结论:良好的平片对诊断腰椎爆裂骨折有一定价值,但要同压缩性骨折鉴别。平片只显示腰椎二维结构,CT能显示腰椎的三维结构,CT对爆裂骨折能作出全面证实和诊断,对小关节紊乱、脱位、间盘改变、椎管继发狭窄,脊髓是否挫伤,CT诊断具有优势。
Objective: To investigate the value of radiography and CT in diagnosing burst fracture of lumbar. Method:Radiographio and clinical findings in 120 patients with burst fracture of lumbar retrospectively analysed. Results:the shape of vertebral body were changed by different degrees through X-ray and CT examinations, its density was reduced and bone fragments were seen to protrude from vertebral body. The distance between pedices of vertebral arch increased or reduced when fracture of vertebral appendages took place. This fracture resulted in compression of vertebral canal,secondary stenosis of vertebal canal, derangement or dislocation of zygapophysial joint, herniation of intervertebral disk and compression of spinal cord. Conclusions: A radiograph of high quality has certain value in diagnosing burst fracture of lumbar, but it is necessary to differentiate it from compression fracture. It dimensional structure displays only a two-dimensional structure of lumbar, but CT displays a three-dimensional one, CT can confirm and diagnose burst fracture in an all-round way. CT occupies a dominant position in diagnosing derangement and dislocation of zygapophysial joint, the change of intervertebral disk,secondary stenosis of vertebral canal and contusion of spinal cord.
