左心与右心衰竭患者血浆肾素-血管紧张素-醛固酮系统心钠素改变的临床意义

摘要 为了研究左、右心衰竭时血浆肾素(PRA)、血管紧张素(AⅡ)、醛固酮(ALD)及心钠素(ANF)浓度的改变,对36例左心衰竭(心功能11级17例、Ⅲ-Ⅳ级19例)、34例右心衰竭患者及33名正常对照中四者的血浆浓度作了放免测定。结果是左心衰患者血浆PRA、AⅡ、ANF及ALD浓度,右心衰患者血浆PRA、AⅡ、ANF浓度均较正常对照升高(P<0.01),而右心衰患者ALD血浆浓度下降(P<0.01),呈现与PRA、AⅡ及ANF的分离现象。左、右心衰竭的患者间及心功能状态不同的患者间相比,四者浓度均有差别。以上四种神经激素参与了心力衰竭的发生、发展过程。 This study evaluated the changes of PRA, A Ⅱ, ALD and ANF concentrations in plasma in heart failure. The concentrations of PRA, A Ⅱ, ALD, ANF in plasma of 36 patients with leftsided heart failure (cardiac functional capacity of 17 cases were Ⅱ, 19 cases were ⅢⅣ), 34 patients with right - sided heart failure and 33 normal controls were detected by PIA.The results show the concentrations of PRA, A Ⅱ, ALD, ANF in plasma of patients with left sided heart failure, the concentrations of PRA, A Ⅱ, ANF of patients with right - sided heart failure all increased than controls (P < 0. 01 ), but ALD concentration of patients with right -sided heart failure decreased apparently (P < 0. 01 )and showed separate phenomenon from PRA, A Ⅱ, ANF. There were also differences between patients of left - and right-sided heart failure and between patients with different condition of cardiac functional capacity. Therefore, the four neurohormones might participate the pathophysiology of heart failure.
