摘要
目的探讨畸变产物耳声发射(DPOAE)在军事噪声致爆震性聋早期监测中的作用。方法将听力正常的炮兵某部男性官兵88例(176耳)随机分成两组,每组44例(88耳),实验组参加火炮实弹射击训练,对照组未参加火炮实弹射击训练。对照组及实验组训练前后进行纯音测听及DPOAE测试,对两组结果进行比较分析。结果实验组训练后80耳中耳传导功能无障碍,其中13耳纯音听阈示感音神经性听力损失,DPOAE检查亦异常;67耳纯音听阈正常,其中15耳DPOAE幅值在4kHz处降低。与训练前、对照组比较,实验组训练后2、4及8kHz纯音听阈升高(P<0.05或P<0.01),以4kHz处升高最为明显;实验组训练后2、3、4、6及8kHzDPOAE幅值降低(P<0.05或P<0.01),其中以4kHz处降低最为明显。结论畸变产物耳声发射在军事噪声致爆震性聋早期监测中有一定应用价值,可对其防护、诊断和治疗提供指导和帮助。
Objective To investigate the role of DPOAE in the early identification of the hearing loss caused by military impact noise. Methods 88(176 ears) artillerymen, with normal otoscopy, pure tone hearing thresholds, acoustic immitance and DPOAE findings, were randomly divided into 2 groups. The test group with 44 young adults (88 ears) participated in the training of cannon shooting, while the control group with 44 young adults (88 ears) received no training. Otoscopy, pure tone audiometry and acoustic immitance were performed on the test group after the training. DPOAE was performed on the artillerymen with normal middle ear. Pure tone hearing thresholds and amplitudes of DPOAE were compared and analyzed. Results DPOAE responses were obtained from 80 otologically normal ears after the training. 13 ears with sensorineural hearing loss showed abnormal DPOAE; 67 ears had normal pure tone hearing thresholds, while 15 ears showed abnormal DPOAEs. The pure tone hearing thresholds at 2, 4 and 8 kHz for the test group after training increased significantly compared with the results before training and that of control group(P〈0.05 or P〈0.01), especially at 4 kHz. The amplitudes of DPOAE at 2,3,4,6 and 8 kHz for the test group after training were reduced significantly compared with the results before training and that of control group(P〈20.05 or P〈0. 01), especially at 4 kHz. Conclusion The results showed that DPOAE had application values in the early identification of the hearing loss caused by military impact noise. DPOAE could offer some directions and assistances in the protection, diagnosis and treatment of impact induced hearing loss.
