影响磁共振冠状动脉成像质量的相关因素分析 被引量:3

摘要 目的探讨影响磁共振冠状动脉成像质量的相关因素。方法85例受检者按心率<70次/min和>70次/min分为两组,分别使用3D-FIESTA磁共振序列进行冠状动脉检查。2组图像质量均分为能用于冠状动脉评价和不能用于冠状动脉评价2类。结果在心率<70次/min组,有92.8%(658/709)的图像可用于诊断,心率>70次/min组,有79.1%(319/403)的图像可用于诊断。结论冠状动脉成像质量与患者心率、呼吸、冠状动脉运动及冠状动脉支架的使用呈负相关,对这些因素的分析有助于提高冠状动脉血管造影的成功率及避免假阳性的诊断。 Objective To investigate the value of breath hold 3 - dimensional fast imaging employing steady state (3 DFIESTA) sequence in the detection of coronary artery stenosis. Methods Forty - three patients with clinically suspect coronary stenoses underwent 3DFIESTA sequence with breath - hold at the end of expiration and the conventiona/coronary angiography was taken as the standard of reference. Target coronary MR images were evaluated the presence of stenoses and image quality were scaled by two observes indenpently. Results The mean image quality score of 302 segments of coronary arteries was 2.6 ±3.5. For the stenoses 〉50% reduction in diameter on DSA, the sensitivity, specificity, positive predictive value and negative predictive value were 86.0% , 98.0% , 93.2%, and 97.5%, respectively. For those segmental stenoses 〈50% on DSA, the sensitivity, specificity, positive predictive value and negative predictive value were 83.0% , 92.0%, 72.0% and 98. 8% , respectively. Conclusion . Breath hold 3 - dimensional fast imaging employing steady state (3DFIESTA) sequence is of value in determination of coronary artery stenoses 〉50% and also may be helpful for those with segmental stenoses 〈50%.
