摘要
目的对糖尿病周围神经病患者进行痛觉传导通路研究,评估其小神经纤维功能。方法对56例有疼痛症状的2型糖尿病患者进行感觉传导速度测定和接触性热痛诱发电位检测;同时进行健康对照组测试。结果糖尿病组感觉传导速度和接触性热痛诱发电位检测与对照组比较,各参数差异均有统计学意义。其中Aδ纤维和C纤维传导速度:糖尿病组分别为(8.6±1.7)m/s和(0.9±0.1)m/s,对照组分别(13.1±2.4)m/s和(1.7±0.3)m/s,差异有统计学意义(t=17.23,21.62,P均<0.05)。糖尿病组中,感觉传导速度异常率与接触性热痛诱发电位异常率比较,差异有统计学意义。结论糖尿病周围神经病中存在小神经纤维的传导异常,提示小神经纤维功能受累。
Objective Pain evoked potential was studied to evaluate the function of small nerve fibers in patients with diabetic peripheral neuropathy. Methods Sensory conduction velocity (SCV) and contact heat evoked potential (CFIEP) were performed in 56 patients with type 2 diabetes mellitus who complained of pain and controls. Results The differences in SCV and the latency of contact heat evoked potential were significant between the diabetic group and the controls. The conduction velocity of A8 fiber and C fiber was ( 8. 6 ± 1.7 ) m/s and (0.9 ± 0. 1 ) m/s in the diabetic and (13.1 ±2.4) m/s and (1.7 ±0. 3 ) m/s in the controls, respectively. Diabetic patients have higher abnormality rates in CHEP than in SCV. Conclusions The conduction velocity of small nerve fibers is slow, indicating that small nerve fibers is involved in patients with diabetic peripheral neuropathy.
