摘要
目的探讨股骨内侧髁骨(膜)瓣逆行转位的解剖学基础。方法在50侧成人下肢标本上,解剖观测膝下内侧动脉、肌间隙血管和隐支、关节支的走行、口径、分支分布及吻合关系。结果膝降动脉隐支与膝下内侧血管在胫骨粗隆下约4.0±1.5cm处吻合(70%),吻合口外径0.7±0.2mm;与肌间隙血管皮支吻合(30%),吻合处在胫骨粗隆下6.6±2.0cm,外径0.7±0.3mm。结论小腿内侧血管吻合构成骨瓣逆行转移的解剖学基础,以该血管吻合网为蒂,通过隐支、关节支逆行供血的股骨内侧髁骨(膜)瓣,可转位修复胫骨中段骨折、骨缺损。
Objective\ To study the anatomy of reversed femoral osteoperiosteal flap.\ Methods\ The course,branches and anastomoses of the medial inferior genicular artery with the saphenous artery and the cutaneous branch of posterior tibial artery were observed on 50 adult lower limbs.\ Results\ A.genu descendens anastomosed with A.genu inferior medialis at 4 0±1 5 cm below the tuberositas tibiae(70%),0 7±0 2 mm in diameter.\ A.genu descendens anastomosed with the cutaneous branch of A.tibiae at 6 6±2 0 cm below the tibal tuberosity(30%),0 7±0 3 mm in diameter \ Conclusion\ Those anastomoses supplied blood reversely to femoral osteoperiosteal flap,it can be transferred for repairing the fracture and defect of middle tibiae.
