多层螺旋CT在汶川大地震脊柱损伤诊断中的应用

摘要 目的探讨多层螺旋CT(MSCT)检查在汶川5.12大地震脊柱损伤诊断中的应用价值。方法对121例地震中脊柱损伤患者行多层螺旋CT容积扫描,在工作站上对图像进行重建处理,观察重建图像对脊柱骨折的显示。结果121例患者共273处骨折,其中颈椎21处、胸椎54处、腰椎176处、骶椎22处,其中附件骨折178处(约占65%),以横突骨折最常见。脊柱骨折以矢状位显示最常用,可清晰、直观显示椎体压缩、骨片移位及椎管变形情况,三维图像可立体显示骨折分离、错位。结论MSCT扫描速度快,图像清晰,能直观、逼真地显示脊柱损伤情况,为临床诊断及治疗提供更加充分的资料,是大型灾害事故中脊柱损伤诊断的最佳手段。 Objective To evaluate the value of muhi - slice spiral computed tomography (MSCT) in the diagnosis of spinal injury during the 5 · 12 Wenchuan earthquake. Methods MSCT volrune scanning was performed in 121 cases with spinal injury during the earthquake, then all the images were reconstructed at the work station, and the spinal fractures were observed in the reconstructed image. Results 121 cases showed 273 spinal fractures, 21 cases with cervical vertebrae, 54 with thoracic vertebrae, 176 with lumbar vertebrae, 22 with sacral vertebrae. The accessory parts fracture was up to 65% ( 178/273 ) , transverse process fracture was the most common injury. Spinal fraetures were showed best in sagittal image, vertebral body compression, bone flap displacement and vertebral canal deformation were observed clearly and intuitively, the separation and dislocation of the fractures could be displayed in the three -dimension(3D) image. Conclusion The scanning speed of MSCT is fast, the images are clear, spinal fractures can be showed intuitively and vividly, sufficient data are provided to clinical diagnosis and treatment. MSCT is the best method for the diagnosis of spinal injury in great disaster and accident.
