摘要
目的:以DNA测序法为标准,探讨PCR熔解曲线法检测乙型肝炎病毒YMDD基因变异的敏感性和特异性。方法:选取35例在拉米夫定抗病毒治疗过程中出现HBV-DNA持续阳性或阴转后再转阳且PCR熔解曲线法示YMDD变异的慢性乙型肝炎患者,另选10例未发生YMDD变异的慢乙肝患者。用DNA测序法进行比较,分析两者检测结果。结果:45例测序样本中有42例两法结果完全一致,总符合率93.3%,另3例结果是:PCR熔解曲线法为不完全变异型,而测序法则是相应的完全变异型。结论:两法的相符率高,PCR熔解曲线法对YMDD变异的检测是一简单易行较实用的方法,适用于临床和基层。
Objective Take DNA sequencing method as the standard, to explore the sensitivity and specificity of HBV-YMDD mutation by PCR melting curve method. Methods Choose 35 samples that the serum YMDD mutation of CHB patients were detected by PCR melting curve method, there patients' HBV-DNA became positive again and they were negative even during Lamivodine therapy. Moreover choose other 10 samples of CHB that have not had the YMDD variation. All detected by DNA sequencing method. Compared with two groups of results that used different methods. Results In the 45 samples ,there are 42 samples whose results are consistent, the total coincidence rate is 93.3 %, another three of results are discrepancy, when used PCR melting curve method,it shows they are not incomplete variation ,but they are complete variation when measured by DNA sequencing method. Conclusion The coincidence rate of two methods is high. It means that using PCR melting curve method to detect YMDD variation is an easy and feasible practical method,and is suitable used in clinical and the basic units.
