摘要
选择2006年10月–2007年6月我科收治的急性左心衰86例,其中男性58例,女性28例,年龄38-78岁,平均60.2±8.5岁。随机分为多巴酚丁胺组和常规治疗组。按急性左心衰常规进行治疗。治疗组给予生理盐水50ml+多巴酚丁胺40mg/iv2–3ug/kg.min-,1次/日,对照组除不用多巴酚丁胺外,基础治疗同治疗组,疗程1周。入院时急诊查TNF–α,疗程结束时复查,观察多巴酚丁胺对TNF–α的影响。结果表明多巴酚丁胺组治疗前TNF–α28.06±5.54ng/ml,治疗后17.82±5.07ng/ml。常规治疗组治疗前TNF–α27.56±4.53ng/ml,治疗后21.26±4.76ng/ml。两组治疗前后相比TNF–α显著下降(P<0.05),治疗后相比多巴酚丁胺组下降更明显(P<0.05)。
86 patients with actute left heart failure between 2006-2007 were selected as case study, among of them, there were 58 cases of male and 28 cases of female, the age were from 38 to 78 years, average age 60.2±8.5 years. The patients were randomly divided into two groups： dobutamine group and control group. The basic treatments are same in two groups. Dobutamine group with NS 50ml＋ dobutamine 40mg/iv2-3ug/kg. min^-, Q. d, total 7 days. TNF-α were measured before treatment and after the treatment of 7 days. The results shows： The level of TNF-α in dobutamine group is 28.06±5.54ng/ml before treatment, and 17.82±5.07ng/ml after treatment. The level of TNF-α in controll group is 27.56±4.53ng/ml before treatment, and 21.26±4.76ng/ml after treatment. Decreasing of the level of TNF-α has significant in the two groups before and after treatment（P〈0.05）, but also there are more significant in dobutamine group after treatment（P〈0.05）. Finally, the conclusion were draw down： Dobutamine can inhibit TNF-α, but the exact mechanism is unclear.
