按揉环跳穴对神经痛大鼠的镇痛效应及其中枢机制研究 被引量:22

摘要 目的:探索按揉环跳穴对神经痛大鼠的镇痛作用及其中枢镇痛机理。方法:采用热测痛法观察按揉环跳穴对坐骨神经结扎所致的神经痛大鼠其疼痛耐受值(s)的变化情况;并以分子生物学方法,研究"以痛为腧"推拿法对神经痛大鼠中枢某些核团的β-内啡肽的变化。结果:在大鼠环跳穴(坐骨神经结扎处)进行按揉后可即刻显著增加其疼痛耐受值,与模型对照组相比具有统计学意义(P<0.05);随着推拿次数的增加,治疗组大鼠疼痛耐受值也逐渐增大,与模型对照组相比有统计学差异(P<0.05)。另外,推拿治疗1周和推拿治疗2周组杏仁核β-内啡肽样免疫阳性反应强度明显强于相应的模型对照组,有显著性差异(P<0.05,P<0.01);而导水管周围灰质(PAG)β-内啡肽样免疫阳性反应强度仅在治疗2周组强于相应的模型对照组(P<0.05)。结论:采用"以痛为腧"推拿方法治疗神经痛大鼠具有较明显的即刻和累积的镇痛作用;推拿治疗神经痛的中枢机制与杏仁核的相关性比与PAG更为密切。 Objective:To research on the analgesic effect of rats with neuropathic pain treated by pressing and kneading on Huantiao (GB30) and its central mechanism of Tuina analgesia. Methods : In this experiment, the method of chronic constriction injury (CCI) is used in making models of rats with neuropathie pain. Apply heat testing method to observe the change of pain tolerance value of the model rats, and research on the change of β - Endorphin in some central nucleus of rats with neuropathic pain treated by pressing and kneading on the tenderness point with the methodology of molecular biology. Results : Pressing and kneading on Huantiao(GB30) (where there had CCI ) can immediately and remarkably increase the pain tolerance value, and has statistic difference with the medel contrast group(P 〈0.05) ; with the increase of times of treatment,the pain tolerance value of the therapeutic group gradually increased,and has statistic difference with the model contrast group (P 〈0.05). Furthermore,the intensity of β - Endorphin immunal positive of rats in amygdaloid body of both the one - week and two - week therapeutic groups was obviously stronger than that of the model contrast group(P 〈 0.05, P 〈 0.01 ). But the intensity of β- Endorphin immunal positive of rats in periaqueductal grey (PAG) in two - week therapeutic group was stronger than that of the model contrast group (P 〈 0.05 ). Conclusion :The pressing and kneading therapy on the tenderness point has an good immediate and accumulative analgesic effect on rats with neuropathic pain. The central mechanism of Tuina analgesia is more related to the amygdaloid body than periaqueductal grey (PAG) .
