摘要
目的:通过观察新生儿脐动脉血气pH值与新生儿结局,探讨其与行为神经测定(NBNA)闻的关系。方法:选择2005年9月至2006年9月分娩的100例足月新生儿进行研究,分为对照组48例和观察组52例,观察组根据病情程度分为重度窒息组15例和轻度窒息组37例,进行新生儿脐动脉血气分析及NBNA评分。结果:对照组与观察组新生儿脐动脉血pH值及BE比较差异均有统计学意义(P〈0.01),对照组与观察组生后4—6d NBNA评分比较差异均有统计学意义(P〈0.01)。结论:胎儿宫内窘迫时,脐动脉血气分析中的pH、BE值及NBNA评分存在一定相关性,是一项判断围生儿产时宫内酸碱代谢状况,评估新生儿近期预后的生化指标。
Objective: To determine the correlation between neonatal arterial cord blood pH and NBNA (neonatal behavioural neurological assessment) through observation. Methods: From September 2005 to September 2006, 100 neonates were randomly divided into two groups. There were 48 patients in the control group and 52 patients in the observation group, who were divided further into the severe asphyxia group of 15 cases and the mild asphyxia group of 37 cases according to their severity. The cord blood pH and NBNA score were assessed for both groups respectively. Results: There was significant difference between the control group and the observation group in terms of pH and base excess (BE) ( P 〈 0. 01 ). The NBNA scores 4 to 6 days pest-delivery showed significant difference between the observation group and the control one (P 〈 0.01 ). Conclusions: For neonates who experienced intrauterine foetal distress, there is acorrelation between cord pH, BE and NBNA score, which allows for the assessment of peri-natal metabolic acidosis. Thus, umbilical arterial pH is a useful biochemical indicator for the prediction of early neonatal prognosis.
