摘要
【目的】研究喉狭窄瘢痕组织的病理变化,了解细胞外基质的变化及意义。【方法】垂直喉部分切除术后2~3月出现瘢痕性喉狭窄10例喉瘢痕组织、非狭窄16例喉瘢痕组织为实验组及10例声带组织为对照组,分别进行Gomori蒺s醛品红法染色、Van Gieson(V.G)染色、苦味酸-天狼猩红染色及免疫组化染色,并在KONTRON IBAS2.5全自动图像分析系统下观察和计算阳性面积。【结果】喉狭窄瘢痕组织中,细胞外基质排列紊乱。与非狭窄喉瘢痕组织、声带组织比较,喉狭窄瘢痕组织中,胶原纤维含量(0.512±0.052)、Ⅰ型胶原纤维含量(0.376±0.015)及纤维连接蛋白表达量(0.472±0.033)明显增多,Ⅰ/Ⅲ型胶原纤维比值(3.32±0.27)明显增大,差异有统计学意义(P<0.05)。与声带组织比较,喉狭窄瘢痕组织中,弹力纤维含量(0.213±0.021)明显减少,差异有统计学意义(P<0.05)。【结论】喉狭窄瘢痕组织中,胶原纤维和纤维连接蛋白等细胞外基质大量沉积。胶原纤维及弹力纤维含量、形态、排列改变,Ⅰ、Ⅲ型胶原数量和比例变化,可能与喉狭窄喉腔弹性差,喉瘢痕僵硬有关。纤维连接蛋白高表达可能与喉狭窄的瘢痕组织过度增生有关。
[Objective] To explore the significance of the change of extracellular matrix in the laryngeal stenosis scar by the study of the pathological features of laryngeal stenosis scar. [Methods] At 2 to 3 months after vertical partial laryngectomy, laryngeal stenosis scar occurred. The patients were divided into the laryngeal stenosis group which were 10 human surgical specimens of laryngeal scar and 16 human surgical specimens of laryngeal scar with non-stenosis, and the control group which were 10 cases of human vocal folds. Collagen fibers and elastic fibers were observed by Gomrori's aldehyde-fuchsin and Van Gieson staining respectively in laryngeal tissue sections. Types Ⅰ and Ⅲ collagen in laryngeal tissues were assessed by picric-sirius red polarimetry staining. Expression of Fn was observed by immunohistochemistry staining. The extracellular matrix was analyzed with KONTRON IBAS 2.5 automatic image analysis system. [ Results ] In laryngeal stenosis scar tissues, the extracellular matrix was arranged disorderly. The density of collagen fibers (0.512 ± 0.052), type Ⅰ collagen fibers (0.376 ± 0.015) and Fn (0.472 ± 0.033 ) in laryngeal stenosis scar tissues was significantly increased compared with that in non-stenosis tissues and human vocal folds tissues; the proportion of type Ⅰ and Ⅲ collagen (3.32 ± 0.27) in the laryngeal stenosis scar tissues was significantly higher than that in the control group (P 〈 0.05). The density of elastic fibers (0.213 ± 0.021) in the laryngeal stenosis scar tissues group was significantly lower than that in the control group (P〈 0.05).[Conclusions] There was over deposition of extracellular matrix in the laryngeal stenosis tissue. The change of contents, shape, range in collagen fibers and elastic fibers, the change of quantity and proportion in type Ⅰ , Ⅲ collagen fibers may be related to larynx cavity elasticity weakening and larynx scar ankylosis of laryngeal stenosis. And the high expression of Fn may be related to the hyper
