摘要
目的调查儿童肠道病毒所致肢体瘫痪的病因、流行病学、临床特征及预后。方法对陕西省西安市儿童医院2001年至2007年收治的儿童肠道病毒所致肢体瘫痪的流行病学、临床特征及相关肠道病毒学检查进行分析。结果23例患儿中男17例（73．9％），女6例（26．1％）；发病年龄以1～3岁为主，为16例（69．6％），发病季节以6～9月为主（13例，占56．5％），农村是该病的高发地域（17例，占73．9％）。多数患儿（13例，占56．6％）有消化道表现，肢体瘫痪多为单侧下肢（20例，占87．0％）；血清免疫学检查以柯萨奇病毒为主（15例，占65．2％），埃可病毒次之（8例，占34．8％），未检出脊髓灰质炎病毒感染的证据。大多数患儿肢体功能3个月内能完全恢复。结论肠道病毒所致肢体瘫痪好发于1～3岁儿童，多表现为单侧下肢瘫痪，柯萨奇病毒感染可能是其主要致病原因，该病预后良好。
Objective To investigate cause, epidemiology, clinical features and prognosis of children＇s motor disturbance of limbs caused by enteroviruses. Methods The data of epidemiology, clinical features, enteroviruses detection results of children with limb paralysis caused by enteroviruses treated in Xi＇ an Municipal Children＇ s Hospital in a period from 2001 to 2007 were analyzed. Results 23 children suffered from limb paralysis, including 17 boys （73.9%） and 6 girls （26.1%）. The disorder was commonly seen in those children aged 1 - 3 years old （16 children, 69.6% ） and in seasons of June to September of a year （13 children, 56.7% ）. The rural area was high prevalence region of the disorder （ 17 children, 73.9% ）. Most of patients （ 13 children, 56.6% ） had gastrointestinal symptoms. The limbs affected were single lower limbs（20 children, 87.0% ）. The serological examination shown that main pathogen was Coxsackie virus （CV） （ 15 cases, 65.2% ） and the next pathogen was echovirus （8 cases, 34.8 % ）. No evidences of poliovirus infection were found. For most children, their limbs＇ function recovered within 3 months completely. Conclusion The limb paralysis caused by enteroviruses is commonly seen in those children aged 1 - 3 years old. Most of the patients manifested single lower limbs＇ paralysis. CV may be main pathogen of the disorder. The prognosis of the disorder is good.
