摘要
目的观察大鼠急性黄磷及其化合物吸入中毒致急性肺损伤/急性呼吸窘迫综合征(ALI/ ARDS)的肺组织病理学改变,探讨血必净注射液的治疗作用。方法将85只健康SD大鼠随机分为正常对照组(5只,质量分数为5%的葡萄糖注射液5 ml/kg)、模型组(20只,5%葡萄糖注射液5 ml/kg)、血必净治疗组(A组,20只,血必净注射液5 ml/kg)、氟美松高剂量治疗组(B组,20只,氟美松注射液5 mg/kg)、血必净+氟美松低剂量治疗组(C组,20只,血必净注射液5 ml/kg+氟美松注射液1 mg/kg)。建立大鼠急性黄磷及其化合物吸入中毒致ALI/ARDS模型,分别于制模后即刻及制模后8h腹腔内注射给药,制模后6、12、24和48 h处死各组大鼠,取肺组织,测定肺湿/干重(W/D)比值,观察肺组织病理学变化。结果模型组大鼠肺组织结构严重破坏,出现明显肺水肿、出血、变性、坏死,且有大量的炎性细胞浸润;肺W/D比值较正常对照组明显增大(P均<0.05)。各治疗组肺组织病理损害程度均较模型组减轻,肺W/D比值明显减小(P均<0.05),减小程度B组>C组>A组。结论黄磷及其化合物吸入中毒致大鼠肺组织病理出现ALI/ARDS样改变,血必净注射液、氟美松对大鼠急性黄磷及其化合物吸入中毒致ALI/ARDS具有治疗作用,二者合用可减少氟美松用量。
Objective To observe the acute toxic pathologic changes of acute lung injury/acute respiratory distress syndrome (ALI/ARDS) in rats after inhalation of yellow phosphorus and its compounds and to study the role of Xuebijing injection (血必净注射液) in protection of the injury. Methods Eighty-five healthy Sprague-Dawley (SD) rats (42 female, 43 male) were made randomly into five groups: a matched control C5 rats, 5% glucose injection (GS) 5 ml/kg], the model of ALI/ARDS (20 rats, 5%GS 5 ml/kg), the treatment group of Xuebijing injection (A group, 20 rats, Xuebijing injection 5 ml/kg), the treatment group of higher dosage of decadron (B group, 20 rats, decadron 5 mg/kg), the treatment group of Xuebijing injection and lower dosage Of decadron (C group, 20 rats, Xuebijing injection 5 ml/kg, decadron 1 mg/kg). After the model of ALI/ARDS after inhalation of yellow phosphorus and its compounds was established, the drug was injected into the rat abdomen immediately once in various groups respectively. After the model was established for 6 hours, 5 rats were killed. After the model was established for 8 hours, the drug was again injected in the remaining rats respectively in various groups. Five rats were killed at 12, 24 and 48 hours respectively at each time point. The lung pathologic changes were observed and the ratio of lung wet/dry (W/D) weight was measured and compared. Results The structure of lung in the model rats was seriously destroyed, showing acute pulmonary edema, hemorrhage, degeneration, necrosis, and a lot of inflammatory cells. The ratio of lung W/D weight in the model group compared with that of the matched control group was much greater (all P〈0.05). In treatment groups, the pathologic changes became milder in various degrees after various treatments in comparisons with those in the model group, and the ratios of lung W/D weight in various treatment groups were significantly lower in various degrees in comparisons with the ratio in the model group
