摘要 目的:探讨在相同牵引重量(9kg)下,不同牵引角度对颈椎生物力学的影响。方法:选择适合条件的10个颈椎病患者,在牵引角度控制仪控制下,分别作中立位、前屈10°、后伸10°位牵引,观察椎体的总体位移。结果:前屈10°位牵引椎体后缘分离较前缘明显,中立位牵引椎间距离增大最明显。结论:颈椎牵引在中立位时对椎间盘的轴向作用最大,前屈位时受力集中在椎体后缘,后伸位转移到后侧小关节。 Objective:To investigate the mechanical effect of traction in treating cervical spondylosis (CS) from different angles with the same weight (9Kg). Methods.. Under the control of traction machine, 10 patients of CS received traction from three different angles: neutral position, 10 degrees in anteflection and backward extension position. The general displacement of centrums was observed after the traction in each patient. Results: The posterior margin separation of centrums was more obvious than anterior margin separation in cases with traction from anteflection position of 10 degrees, while the most increases of intervertebral space occurred in cases with traction from neutral position. Conclusion: Cervical traction has the strongest stress on intervertebral discs when it is performed in neutral position. As in anteflection position, the stress is concentrated on posterior margin of centrums, and the stress shifts to posterior small joints when in backward extension position.
