摘要
目的探讨特殊类型主动脉瓣置换术中主动脉瓣环切除及重建术的应用,评价其临床效果。方法18例患者,在全麻低温冠脉逆冠体外循环下行瓣环切除瓣环扩大重建术及瓣膜置换术。术前及术后6个月经彩色多普勒检查,评价血流动力学。结果全组无严重手术并发症。有效瓣膜开口面积指数(EOAI)、最大跨瓣压差(△P)和左室舒末直径(LVEDD),术后6个月均较术前明显改善,达到临床治疗标准。结论主动脉瓣环切除及重建术在特殊类型的主动脉瓣膜置换中安全可靠,操作简单,临床效果满意。
Objective To investigate the clinical application of the excision and reconstruction of aortic vavle ring in the special patient who underwent aortic vavle replacement. Methods All 18 cases were performed the excision and reconstruction of aortic vavle ring under general anesthesia and moderate hypothermia bypass with retro-cardioplagie.Before and six months after operation, we evaluate the blood flow dynamics by Colored Dopple. Results Entire group have not serious surgery complication. The relative index including.The Peak Transprosthetic Gradients,EOAI,LVEDD has shown improvement after 6 months. Conclusion The aorta vavle ring excision and the aortic root reconstruction in the special type aorta valve replacement is safe and reliable, the operation is not complex, and the clinical effect is satisfied.
