摘要
目的探讨非酒精性脂肪肝(NAFLD)发病与患者血清瘦素、肝脏瘦素受体表达及其基因Lys109Arg多态性、体重指数(BMI)、血脂紊乱的关系。方法将96例经肝穿活检诊断为NAFLD的患者分为轻、中、重三度,同时选择50例肝穿活检非脂肪肝者为正常对照,检测其血清瘦素、肝组织内瘦素受体mRNA表达情况、瘦素受体基因LyS109Arg多态性、空腹血糖、血脂,计算BMI。结果NAFLD组与对照组之间血清瘦素水平、肝脏瘦素受体表达、BMI及血脂紊乱比较有显著差异(P〈0.05或0.01),NAFLD患者随病变程度加重,血清瘦素水平有逐渐增高的趋势,且呈正相关关系(P〈0.05)。对照组与NAFLD组及病变不同程度脂肪肝组之间瘦素受体基因LyS109Arg多态性分布差异均无统计学意义。结论血清瘦素、BMI增高及血脂紊乱是NAFLD发病的危险因素,NAFLD病变程度与血清瘦素水平呈正相关,体内高瘦素对瘦素受体表达有下调作用,这可能是引起瘦素抵抗的机制之一。无证据提示瘦素受体基因LYS109Arg的多态性参与脂肪肝的发生及发展。
Objective To investigate the relationship among serum leptin, expressions of OB2R mRNA in liver, gene Lys109Arg polymorphism, body mass index(BMI)and dyslipidema in patients with nonalcoholic fatty liver disease(NAFLD).Methods Ninty six patients with NAFLD were divided into mild, moderate and severe NAFLD groups according histological examination results of the liver. Fifty healthy volunteers were chosen as the normal control (NC) group. Leptin, expressions of OB2R mRNA in liver, gene Lys109Arg polymorphism and lipid levels were measured in the 96 patients with NAFLD and the BMI calculated. Results Leptin, BMI and dyslipidemia showed significant differences between NAFLD and NC groups(P〈0. 05or0. 01).Leptin index increased with the exacerbation of NAFLD, was positively correlated with the severity of NAFLD(P〈0. 05).The distribution of leptin receptor Lys109Arg polymorphism had no significant difference between NAFLD and NC groups. Conclusion Leptin, expressions of OB2R mRNA in liver, gene Lys109Arg polymorphism ,BMI and dyslipidemia can be important risk factors of NAFLD. Serum leptin level is positively correlatedwith the severity of NAFLD. Hight leptin level can reduce expressions of OB2R mRNA in liver, that may be the leptin resistance. There is no evidence showed that the gene Lys109Arg polymorphism is concerned with NAFLD.
