小儿呼吸道合胞病毒肺炎297例中西医治疗方案成本及效果分析 被引量:3

Cost-effectiveness analysis of traditional Chinese medicine (TCM) and western medicine therapeutic schemes for 297 cases of child respiratory syncytial virus pneumonia.

摘要 目的评价中西医两种方案治疗呼吸道合胞病毒肺炎的疗效和临床经济学意义。方法研究病例来源于北京儿童医院、南京中医药大学附属医院、天津中医药大学第一附属医院、河南中医学院第一附属医院和广东省中医院5所医院2005年1月至2007年2月确诊为呼吸道合胞病毒肺炎并经中医辨证诊断为风热闭肺或痰热闭肺证的患儿297例,采用中心分层区组随机化方法通过计算机制定随机表,按照病例编号所对应的分组,分为中药观察组148例和西药对照组149例。比较其疗效和成本差异,进行成本-效果比及敏感度分析。结果观察组愈显率90.54%,对照组愈显率81.88%,两组疗效差异有统计学意义(χ2=2.2655,P<0.05),观察组平均成本为3424.05元,对照组平均成本为3716.64元,成本-效果比观察组低于对照组;敏感度分析观察组成本-效果比仍低于对照组。结论中药试验方案治疗小儿呼吸道合胞病毒肺炎的临床经济学评价优于西药对照方案。 Objective To evaluate the therapeutic effect and clinical economics between TCM and western medicine therapeutic schemes in treatment for child respiratory syncytial virus pneumonia. Methods A total of 297 cases of child respiratory syncytial virus pneumonia from January 2005 to February 2007 with TCM syndromes of wind and heat invasion of lung or accumulation of phlegm-heat in lung came from 5 clinical centers, Beijing Children's Hospital, the Affiliated Hospital of Nanjing University of TCM ,the First Affiliated Hospital of Tianjin University of TCM,the First Affiliated Hospital of Henan College of TCM and Guangdong Provincial Hospital of TCM, who were randomly divided into the TCM group ( n = 148) and western medicine controlled group ( n = 149) by the center stratified randomization method. Therapeutic effect and costs of two groups were collected and compared, and cost -effectiveness analysis and sensitivity analysis were carried out. Results In the test group the cured-significant effective rate was 90. 54%, while in the control group the cured-significant effective rate was 81.88%, there being differences in efficacy in statistics between test group and controlled group ( Х^2 = 2. 2655, P 〈 0. 05 ) ; the average cost for the test group was 3424. 05 ynan, for the control group 3716. 64 ynan, the cost -effectiveness ratio for test group being lower than controlled group, the result of sensitivity analysis being stable. Conclusion In treating respiratory syncytial virus pneumonia,TCM therapeutic scheme is better than westem medicine controlled groups in health economics.
