摘要
目的分析和探讨视网膜母细胞瘤(Rb)临床诊治特点。方法对1996～2005年收治的317例(362眼)Rb患儿临床资料进行回顾性分析。结果Rb患儿初诊症状最常见的是白瞳症,占81.39%,初诊年龄平均为30.18个月,主诉时间平均为117.86d,眼内Rb肿瘤厚度平均为10.71mm。有家族史者占总体的1.89%;单眼为0.37%,双眼为10.42%。Rb分期:眼内期占总体的55.33%,青光眼期为10.00%,眼外期为32.67%,远处播散期为1.00%,自发萎缩者为1.33%。治疗包括传统治疗方法和保守治疗方法,放弃治疗者占总体的9.39%,保守治疗者占治疗患儿的12.50%,眼球摘除者占治疗患儿的91.46%。结论Rb患儿就诊较晚且病情严重,眼内Rb肿瘤体积较大,眼外期患儿所占比例较大。目前Rb保守治疗率较低,眼球摘除率较高。
Objective Retinoblastoma is the most common primary malignant intraocular tumor in children.It severely threaten their visual acuity and even life of patient.This study was to analyze the clinical characteristics of diagnosis and treatments for retinoblastoma.Methods The clinical document of 317 cases（362 eyes）with retinoblastoma confirmed in Zhongshan Ophthalmic Center from 1996 to 2005 were retrospectively analyzed.Results In all the 362 eyes,leucocoria was the most common manifestation with the primary diagnosis rate 81.39% based on this symptom（258 eyes）.The average age of primary diagnosis was 30.18 months,and the course from the main complaint was 117.86 days.The thickness of intraocular tumor was 10.71 mm.In all the 317 cases,6 cases（1.89%）showed the positive family history.1 cases（0.37%）presented the unilateral retinoblastoma and 5 cases（10.42%）for biocular retinoblastoma.Retinoblastoma staging：166 eyes（55.33%）showed the intraocular stage of retinoblastoma and glaucoma stage was 30 eyes（10.00%）,extraocular stage 98 eyes（32.67%）,metastasis stage 3 eyes（1.00%）and spontaneous atrophy stage 4 eyes（1.33%）.Management of retinoblastoma included traditinonal therapeutical method and conservative approach with the abandoning treatment rate 9.39%（34 eyes）and conservative treatment rate 12.50%（41 eyes）and eyeball removal rate 91.46%（300 eyes）.Conclusion These results indicate that retinoblastoma patients have bigger tumor volume and more proportion of extraocular stage because of the long course.The early diagnosis and prompt treatment can reduce the eyeball removal rate.The study of effective approach to treating retinoblastoma is helpful for retinoblastoma patient.
