摘要
目的研究腰麻-硬膜外联合麻醉在急诊剖宫产手术的应用。方法选择60例需急诊行剖宫产术的健康临产妇,随机分为硬膜外麻醉组和腰麻-硬膜外联合麻醉组,每组30例。麻醉过程中监测血流动力学;麻醉效果记录阻滞平面达T8水平的时间及注射局部麻醉药到胎儿娩出的时间;新生儿娩出1min和5min的Apgar评分;恶心呕吐、头痛、低血压等不良反应,并于出院前随访患者有无神经系统症状。结果两组术中血压、心率、脉搏、血氧饱和度的变化,不良反应及新生儿娩出1min和5min的Apgar评分间差异均无统计学意义(P>0.05)。同硬膜外麻醉组相比,腰麻-硬膜外联合麻醉组麻醉起效时间及注射局部麻醉药到胎儿娩出的时间间差异有统计学意义(P<0.05)。结论腰麻-硬膜外联合麻醉可视为急诊剖宫产术的首选麻醉方法。
Obsjective To research applications of lumbar spinal anesthesia-epidural anesthesia in emergency Cesarean section surgery. Methods Taking 60 healthy pregnant women who are brought to bed and required emergency Cesarean section surgery. They were randomly divided into two groups equally for epidural anesthesia and lumbar spinal anesthesia-epidural anesthesia. Blood dynamics was monitored in the course of anesthesia. The times of block levels to T8 and injected anesthetic to the fetus delivered were recorded in the anesthesia effort table; newborns delivered 1 min and 5 min of the Apgar score; nausea and vomiting, headache, low blood pressure and other adverse reaction. Response and follow-up patients whether there were nervous system symptoms before they discharged from hospital. Results. Two groups in blood pressure, heart rate, pulse oxygen saturation changes, adverse reactions and their newborns delivered one minute, five minutes of the Apgar score were not significant （ P 〉 0.05）. Compared with epidural anesthesia,lumbar spinal anesthesia-epidural anesthesia group and the onset time of narcotic local anesthetic injection to the fetus delivered significantly shorten the time, anesthetic effect more reliable （ P 〈 0.05）. Conclusion Lumbar spinal anesthesia-epidural anesthesia emergency Cesarean section can be seen as the preferred method of anesthesia.
