摘要 【目的】评价贝尼地平对原发性高血压(EH)患者大动脉内皮功能及粥样硬化的影响。[方法]EH患者35例予以贝尼地平(4~8mg/d)治疗12周,另设与之匹配的健康对照组20例,采用高分辨率超声技术,检测两组的颈动脉内一中膜厚度(IMT)、颈总动脉内径(ID)、肱动脉内皮依赖性舒张功能(FMD)及颈动脉斑块的Crouse积分。并观察EH组治疗前后血压和上述血管超声指标的变化。【结果】与对照组比较,EH组肱动脉FMD减退,颈动脉ID减少,IMT增厚,Crouse积分增加(P〈0.01)。贝尼地平治疗12周后,降压疗效明显,颈动脉IMT明显减低(P〈0.01),ID增加(P〈0.05),肱动脉FMD明显改善(P〈0.01)。【结论】贝尼地平降压疗效显著,能延缓动脉粥样硬化发生发展,逆转大动脉功能和结构异常,改善血管内皮功能。 [Objective] To investigate the effect of treatment with benidipine on endothelial function and atherosclerosis progression of large arteries in essential hypertension(EH) patients. [Methods]Thirty five patients with EH were treated with benidipine for 12 weeks, and 20 healthy subjects were collected as control group ,whose age and sex were matched with the therapeutic group. The intimal-medial thickness (IMT) and internal diameter(ID) of carotid artery, humeral arterial endothelium dependent vasodilatation(FMD) and carotid plaques were measured with high resolution ultrasonography. At the same time, the blood pressure was measured before and after the treatment. [Results] There was worse FMD, narrower ID of carotid artery, thicker MIMT and higher Crouse score in the patients with EH compared with the control( P 〈0.01). The blood pressure and IMT were significantly decreased ( P 〈0.01), however ID and FMD was markedly increased ( P 〈0.05) after 12 week-treatment with benidipine in EH. [Conclusion] Benidipine can not only control blood pressure, but also slow down atherosclerosis progression, reverse structure of large artery and improved endothelial function in EH patients.
