摘要
目的探讨腔内激光治疗(EVLT)下肢浅静脉曲张复发原因。方法回顾性分析2003年9月—2007年9月治疗的317例患者(446条肢体),其中单纯行EVLT 130例197条肢体,行大隐静脉高位结扎+EVLT 187例249条肢体,随访304例患者分析术后下肢浅静脉曲张复发原因。结果随访率95.9%,平均随访(19±4)月,单纯应用EVLT复发率为21.8%(43/197);高位结扎+EVLT复发率为8.8%(22/249),二者比较差异有统计学意义(P<0.01)。术后不同时间复发率比较,差异亦有统计学意义(P<0.05);全程及大腿主干复发率差异有统计学意义(P<0.05),但小腿交通支及静脉团复发率差异无统计学意义(P>0.05)。结论EVLT结合高位结扎大隐静脉根部及分支、病变的交通支静脉可有效降低复发率。
Objective To investigate the reasons of recurrence of superficial vein varicose of lower limb, which treated with endovenous laser treatment （EVLT）. Methods Retrospective analysis of 317 cases （446 limbs, from Sept, 2003 to Sept, 2007） with superficial vein varicose of lower extremity. 130 cases of them （197 limbs） were treated with simple EVLT, and 187 of them （249 limbs） were treated with ELVT combined with great saphenous vein high ligation. 304 of them were followed up and analyzed the recurrence reason. Results The rate of follow up was 95.9% （304/317） ,and the average time of follow up was （19 ± 4） months. It demonstrated that the recurrence rate was 21.8% （43/197） in simple ELVT group, and 8.8（22/249） in EVLV combined with high ligation group,There was statistical significance between these two groups （ P 〈 0101 ）, the same result comparing between recurrence time （ P 〈 0.05） .There was significance difference in the position of whole range and thigh （ P 〈 0105） ,otherwise,tbere was no significance difference in the position of leg communicating branches and vein mass（ P 〉 0.05）. Conclusion It provided that EVLT combined with high ligation of great saphenous vein and dealing with the communicating branches lesion could effectively reduce the recurrence rate.
