非拔牙矫治恒牙列早期安氏Ⅱ类Ⅰ分类错殆的临床体会

摘要 目的探讨利用功能性矫治器、直丝弓矫治技术治疗安氏Ⅱ类Ⅰ分类错耠,并比较矫治前后软、硬组织头影测量的变化。方法对30例安氏Ⅱ类Ⅰ分类错(牙合)的恒牙列早期临界病例采用非拔牙矫治,进行双期治疗。第一期利用功能性矫治器抑制上颌骨生长,推磨牙向后,扩大上、下牙弓,引导下颌正常生长;第二期采用直丝弓矫治器治疗技术,适当配合Ⅱ类牵引,矫正下颌后缩,直至达前牙的覆盖、覆(牙合)正常。对两期相关的软、硬组织进行X线头影测量。结果利用功能性矫治器、直丝弓矫治器治疗安氏Ⅱ类Ⅰ分类错(牙合),使颌骨、牙齿、软组织有不同程度的改变。结论利用直丝弓矫治技术,选择合适的功能矫治器可以使恒牙列早期安氏Ⅱ类1分类错(牙合)邻界病例非拔牙矫治获得满意的效果。 Objective To analyze the effects of orthotherapy by functional orthotic appliance and edgewise-orthotic skill on Angle class Ⅱ division Ⅰ malocclusion. Methods 30 patients with Angle class Ⅱ division Ⅰ malocclusion were treated by non-extraction orthotherapy. During the first period, methods curbing maxillary, moving molars distally, extending upper/lower teeth edgewise and having medibular growth normally were taken, while during the second period, the edgewise appliance combining proper traction was used. Lateral head cephalograms were taken during the two periods. Results The orthotherapy of Angle class Ⅱ division Ⅰ malocclusion was taken by functional orthotic appliance and edgewise appliance. Maxilla, mandible, hard and soft tissues were improved at various degrees. Conclusion Non-extraction orthotherapy of Angle class Ⅱ division Ⅰ malocclusion may get satisfactory effects when edgewise-orthotic skill and proper functional orthotic appliance are used.
