摘要
目的:探讨类风湿性肺间质病变的CT表现,提高诊断水平。材料和方法:搜集经临床、病理证实的类风湿性肺间质病变17例,回顾性分析其CT表现。结果:17例中CT表现两肺周围部弥漫性磨玻璃样病变14例,其中小叶间隔增厚7例,支气管血管束增厚3例;胸膜粘连、增厚5例,其中伴有少量胸腔积液1例;两上肺出现大小不等的结节,有空洞形成,两肺间质纤维化1例;两肺纹理增粗、扭曲、紊乱,夹杂网点状阴影2例。结论:类风湿性肺间质病变CT表现以肺间质纤维化为主,表现复杂,缺乏特征性,CT必须紧密结合临床病史,实验室检查及其它检查,才能明确诊断。
Purpose： To study the CT findings and imprive CT diagnostic accuracy of the lung in patients with rheumatoid arthritis. Materials and Methods： 17 cases of the lung in patients with rheumatois arthritis were Studied. All patients were proved by clinic and pathology. Results： Among 17 cases , CT showed that both bilateral lungs had diffuse ground-glass opacities in 14 cases. Of these , there were thickened lobular septa in 7 cases , thickened brinchivascular bundles in 1 cases, pleural thickening and adhesion in 5 cases ,with a little of pleural effusion in 1 case . There were various size of pulmonary nodules with cavity formation and pulmonary interstitial fibrosis in 1 case , Both bilateral lungs had thickening , twisted ,disordered pulmonary markings with reticular opacities in 2 cases. Conclusions： CT findings of the lung in patients with rheumatoid arthritis are very complex and mainly pulmonary interstitial fibrosis , but onospecific. So CT findings must be combined with clinical data and laboratory tests and other examinations , that makes a definite diagnosis.
