摘要
目的总结经瞳孔温热疗法(transpupillary thermotherapy,TTT)联合光动力学疗法(photodynamic therapy,PDT)治疗脉络膜新生血管形成(choroidal neovascularization,CNV)的效果。方法临床确诊的脉络膜新生血管患者15例24只眼,平均年龄59岁。其中年龄相关性黄斑变性合并脉络膜新生血管14只眼,病理性近视合并脉络膜新生血管10只眼。在TTT治疗3个月复查后,针对仍存在的CNV行PDT治疗。其中19只眼接受2次PDT治疗,5只眼接受3次PDT。结果8只眼(33.3%)视力提高>2行,12只眼(50.0%)视力波动在1行,4只眼(16.7%)视力下降>2行。眼底荧光素造影检查联合眼底吲哚菁绿血管造影检查显示,渗漏停止7只眼(29.2%),渗漏减少14只眼(58.3%),渗漏无明显变化2只眼(8.3%),1只眼(4.2%)出现新的新生血管出血。光相干断层扫描检查,出血水肿吸收13只眼,无明显变化10只眼,加重1只眼。结论TTT联合PDT是治疗脉络膜新生血管维持视力稳定的有效方法。
Objective To summarize the therapeutic effect of transpupillary thermotherapy (TTT) combined with photodynamic therapy (PDT) for the patients with subfovel ehoroidal neovaseularization(CNV). Methods The elinieal data from 15 patients(24 eyes) indieated that, 14 eyes were with age-related maeular degeneration, and 10 eyes were with pathologic myopia. All of the patients had undergone TTT, and after 3 months were given PDT, the follow-up period lasted 3-6 months. The average age of patients was 59. Two eyes underwent twiee PDT,and 5 eyes of PDT 3 times . Results In the final follow-up examination, eompared with the eondition before TTT, the visual aeuity inereased 〉 2 lines in 8 eyes (33.3%), ehanged + 1 line in 12 eyes(50.0% ) , and deereased 〉 2 lines in 4 eyes(16. 7% ) ; the results of fundus fluoreseein angiography(FFA) eombined with indoeyanine green angiography(ICGA) showed that the leakage of CNV stopped in 7 eyes(29.2% ) , redueed in 14 eyes(58.3% ) , remained in 2 eyes(8. 3% ) , and 1 eye(4. 2% ) had new CNV. The result of optie eoherent tomography (OCT) showed that the edema was alleviated in 13 eyes, remained in lOeyes, aggravated in 1 eye. Conclusions After transpupillary thermotherapy, given photodynamic therapy is an efficient way to stop patients losing correct visual.
