应用微创内固定系统治疗股骨Vancouver C型骨折

Treatment of Vancouver type-C peri-prosthetic femoral fractures with less invasive stabilization system

摘要 目的分析采用有限切开复位微创内固定系统(LISS)固定治疗Vancouver C型股骨骨折的临床疗效。方法对2002年3月至2006年11月收治的8例Vancouver C型股骨骨折患者采用LISS治疗,其中男3例,女5例,骨折发生时平均年龄为67岁。骨折发生在髋关节置换术后8个月~5年,对这些患者进行临床评估。结果随访时间为8~38个月,平均17个月。8例骨折愈合时间平均为4.3个月,Harris评分平均83分(76~92分)。所有患者没有使用同种异体骨条或自体骨植骨,无并发症出现。结论LISS钢板在治疗Vancouver C型股骨骨折时显示出相对于其他类型钢板的优势,尽可能保护了骨水泥壳,在此型骨折中锁钉显示出良好的固定能力,治疗效果良好。 Objective To analyze treatment of Vancouver type-C femoral fractures by open reduction and internal fixation with less invasive stabilization system (LISS). Methods Eight patients with Vancouver type-C femoral fracture were treated with LISS. They were 3 men and 5 women, with an average age of 67 years at the time of fracture. The fractures occurred 8 months to 5 years after a hip arthroplasty. The patients were assessed clinically. ResuIts Follow-up lasted 8 to 38 months, averaging 17 months. The 8 fractures healed uneventfully after an average of 4.3 months. No cortical strut allograft or autogenous bone was utilized and no complication was found. Conclusions LISS offers advantages over other types of plates in the treatment of type-C femoral fractures. It has the potential to preserve the cement mantle, and the locked screws have good pullout resistance. The therapeutic efficacy is fine.
