摘要 目的:探讨肝细胞肝癌(简称肝癌)患者外周血中调节性T细胞的表达状态、临床意义以及肝癌切除对其表达的影响。方法:利用流式细胞仪检测肝癌患者及健康对照者外周血中调节性T细胞群的表达频数,分析其与肝癌病理特征及肿瘤TNM分期的关系,并进一步观测肝癌切除前后外周血CD4+CD25+Foxp3+T细胞频数的动态变化。结果:肝癌患者外周血中CD4+CD25+Foxp3+、CD4+Foxp3+、CD4+CD25+T细胞频数均较健康对照组显著增加。调节性T细胞群表达和肿瘤数目、血管侵犯及TNM分期均明显相关。肝癌切除术后第1天调节性T细胞频数较术前明显下降;术后1周时调节性T细胞表达出现最高峰。结论:肝癌患者调节性T细胞群表达增加;调节性T细胞可能主要在肝癌病程演进的较早阶段发挥作用;肝癌切除可能降低肿瘤相关性调节性T细胞的表达。 Objective:To evaluate the presence,clinical significance of the peripheral blood regulatory T cells in hepatocellular carcinoma(HCC) patients and study the influence of liver cancer resection on the presence of CD4^+ CD25^+ Foxp3^+ regulatory T cells.Methods:By flow cytometry,the presences of peripheral regulatory T cell subsets in HCC patients and healthy donors were evaluated.The relationship between the presence of regulatory subsets and HCC clinicopathologic characteristics and TNM stages were analyzed.The dynamic presences of peripheral CD4^+CD25^+Foxp3^+ T cells before and after liver cancer resection were also studied.Results:The percentages of peripheral blood CD4^+CD25^+Foxp3^+,CD4^+Foxp3^+ and CD4^+CD25^+ T cells of HCC patients were significantly higher than those of the healthy donors.The percentages of peripheral regulatory T lymphocyte subsets significantly correlated with tumor number,vascular invasion and TNM stages respectively.The frequency of peripheral regulatory T cells on the first day after liver cancer resection were significantly lower than that before operation.The presence of regulatory T cells reached its climax one week after operation.Conclusion:The presences of regulatory T lymphocyte subsets in HCC patients were significantly elevated.The peripheral regulatory T cells may play a role predominantly in the early stage of HCC progression.Liver cancer resection might reduce the peripheral tumour-related CD4^+CD25^+Foxp3^+ regulatory T cells.
