摘要
目的探讨不同营养方式对肝硬化肝癌大鼠肝部分切除术后免疫功能的影响。方法将成功建立肝硬化肝癌大鼠模型的大鼠随机分为肠外营养组(PN组)、肠内营养组(EN组)、肠内肠外混合营养组(PN+EN组,PN和EN供能各50%)、常规经口饮食组(R0组),应用Higgins法行大鼠肝部分切除术,术后给予上述不同营养方式1W,分别于术前、术后第1、3、7、14天:ELISA法检测大鼠血清的IL-2、IL-6、INF—γ、TNF-α;流式细胞仪检测全血的CD3、CD4、CD8、CD4/CD8、NK;单纯琼脂扩散法检测血浆IgA、IgM、IgG。结果PN组营养对大鼠术前、术后免疫功能无明显影响(P〉0.05);EN组营养增强大鼠术后免疫功能(P〈0.05);PN+EN组营养同样能增强大鼠术后免疫功能(P〈0.05),但效果差于EN组(P〈0.05)。结论EN营养可明显增强大鼠术后免疫功能,PN+EN营养次之,PN无明显免疫增强功能。
Objective To explore the effects of different nutritional pathway on immune function in rats with hepatic cirrhosis and hepatoma undergoing partial hepatectomy. Methods The successfully established model of rats with hepatic cirrhosis and hepatoma undergoing partial hepatectomy by Higgins'skill were randomly divided into parenteral nutrition group (PN group), enteral nutrition group (EN grooup), PN+EN group and routinely oral group (RO group).The serum levels of IL-2, IL-6, INF- γ,TNF- α and the percent of CD3^+,CD4^+,CD8^+,NK, CD4^+/ CD8^+ and the plasma levels of IgA, IgM, IgG were observed respectively at different times of perioperation. Results There were no differences in immune function between PN group and RO group (P〉0. 05), but signifcant differences between EN group and PN group (P〈0.05). There were also differences in immue function between PN+EN group and PN group (P〈0.05). Conclusion The nutritional pathway of PN+EN nutrition can enhance immune function in rats with hepatic cirrhosis and hepatoma undergoing partial haptectomy and the higher effects in enteral nutrition, but no effects in parenteral nutrition.
