股静脉梗阻性病变侧支血管建立的CT评价

摘要 目的探讨股静脉梗阻性病变侧支循环建立的途径。方法采用下肢直接静脉造影的方法对36例病人进行检查。应用容积再现(VR)显示和判断病变侧支循环图像。结果股静脉梗阻所致的侧支循环主要包括两类:第一类为深静脉之间的吻合支形成,同侧闭孔静脉为主要深静脉吻合通路(30例,83.33%);第二类为深静脉与浅静脉之间的吻合支形成。深静脉和浅静脉的吻合途径主要是腹壁静脉(30例,83.33%),旋髂静脉(18例,50%),阴部外静脉(12例,33.33%),其它浅静脉(12例,33.33%)。结论VR技术对判断股静脉梗阻性病变侧支循环是非常有价值的。 Objective To evaluate the pathways of the collateral vessels in patients with femoral vein obstruction. Methods The lower extremities direct CT venography was performed in 36 patients. Volume rendering(VR) was applied to display and judge the images of the collateral vessels. Results The collateral vessels of femoral vein obstruction were found to be two kinds. The one group was the anastomosis vessels formed between deep veins, and the main anastomosis vessel was obturator vein (33 cases, incidence rate 83.33%). The other kind of anastomosis occurred between the deep veins and superficial veins, and the main anastomosis path was epigastrica vein (30 cases, 83.33%), circumflexa ilium vein (18 cases, 50%), external pudendal vein ( 12 cases, 33.33%), or other veins (12 cases, 33.33%). Conclusion VR is highly valuable in judging the pathways of the collateral vessels in patients with femoral vein obstruction.
