心率变异性新指标对左室功能的评定价值

摘要 目的在已知有心脏疾病并伴有左室收缩功能降低的患者中,评定心率变异性(HRV)频域分析新指标。方法病变组:66例左室收缩功能降低者;对照组:64例心脏结构功能正常且无严重冠脉狭窄者。用5min心电图测定心率变异性频域中极高频成分并生成新指标VHF指数(VHFI)。结果病变组VHFI显著高于对照组(19.17±13.35vs11.37±10.77,p<0.05);VHFI与左室收缩功能呈负相关(R=-0.612);以VHFI>15为阳性,VHFI<15为阴性,对于左室收缩功能降低的预测敏感性和特异性分别为57.58%和78.13%。结论VHFI可以作为有价值的指标,用于急诊快速评定左室功能。 Objective We studied a new index based on frequency-domain analysis of Heart Rate Variability (HRV) and tested that on patients with known cardiac diseases and reduced left ventricular systolic function. Methods We divided patients (n = 130) to study group (n = 66) consisting of patients with decreased left ventricular systolic fimction, and control group (n = 64) consisting of patients with normal heart structure and function and without severe coronary 'artery stenosis (〈50%). We calculated Very High Frequency (VHF) component based on five-minute ECG recordings and used a new VHF Index (VHFI). Results VHFI in study group was significant high than that in control group (19.17 ± 13.35 vs 11.37 ± 10.77 ). VHFI had correlation with ventricular systolic function (R=-0.612). We defined the positive test as VHFI〉15 and negative test as VHFI〈15 and obtained sensitivity of 57.58% and specificity of 78.13% for predicting decreased left ventricular systolic function. Conclusion We found out that VHFI has potential to be a useful tool for quick evaluation of left ventricular function in emergency setting.
