摘要
目的比较有睡眠障碍和无睡眠障碍的注意缺陷多动障碍(ADHD)患儿的认知行为特征,评估睡眠障碍与ADHD患儿认知行为功能之间的关系。方法选取2005年7月-2007年9月在广东省妇幼保健院儿童保健心理门诊确诊为ADHD的7～10岁患儿112例。男95例,女17例。根据儿童睡眠障碍诊断标准,分为无睡眠障碍ADHD组(n=55)和有睡眠障碍ADHD组(n=57)。采用韦氏儿童智力量表、韦氏记忆量表、数字划消测验和Conners儿童行为问卷,分别对二组患儿的智力、记忆力、注意力和行为问题进行测试与对比分析。结果1.有睡眠障碍ADHD组患儿全量表智商显著低于无睡眠障碍ADHD组,其中尤以操作智商降低为明显(P<0.01),二组间言语智商比较差异无统计学意义(P>0.05);2.有睡眠障碍ADHD组患儿的长时记忆、记忆商和数字划消得分均显著低于无睡眠障碍ADHD组(Pa<0.01),数字划消失误率显著高于无睡眠障碍ADHD组(P<0.01);3.有睡眠障碍ADHD组患儿的学习问题、冲动/多动、焦虑和多动指数因子得分均显著高于无睡眠障碍ADHD组(Pa<0.01),品行问题和心身障碍因子得分二组间比较差异无显著性(P>0.05)。结论有睡眠障碍ADHD患儿的认知行为受损更严重。
Objective To compare the cognition and behavior characteristics in children with attention deficit hyperactivity disorder （ADHD） combined with or without sleep disorder （SD） and assess the potential relationship between SD and cognitive behavioral function in children with ADHD. Methods One hundred and twelve children of 7 - 10 years old （95 boys and 17 girls） were selected from department of children＇s psychology health care of Guangdong province women and children hospital from Jul. 2005 to Sep, 2007 ,who were divided into ADHD without SD group （ n = 55 ） ,ADHD with SD group （ n = 57 ） on the basis of diagnosis criteria of SD. A series of cognitive and behavioral tests were performed including the Chinese version of Wechsler intelligence scale for children, Wechsler memory scale, number cancellation test （NCT） and Conrier＇s parent rating scale to determine the intelligence, memory, attention and behavior of all patients, Results 1. Compared with ADHD without SD group,the full intelligence quotient in ADHD with SD group significantly decreased, noticeably performance intelligence quotient （ P 〈 0.01 ）, No statistically significant was found between the 2 groups on verbal intelligence quotient （ P 〉 0.05 ）. 2, In Wechsler memory scale and NCT, the children in ADHD with SD group were significantly lower on the scores of long term memory, memory quotient NCT scores and significantly higher on the scores of NCT error rate than those in ADHD without SD group （Pa〈 0.01 ）. 3. The results of Conner＇s parent rating scale showed that children in ADHD with SD group had more hyperactivity, study problem, impulse and anxiety problems compared with those in ADHD Without SD group （Pa〈0.01 ）. No significant differences were found between the 2 groups on conduct problem and psychosomatic disorder （Pa〉 0.05 ）. Conclusion The children with ADHD and SD have much worse disorders Of cognition and behavior.
