探讨流式HLA-B27平均荧光强度用于原发强直性脊椎炎及继发性脊椎炎的鉴别诊断 被引量:1

摘要 目的探讨B27平均荧光强度用于原发性强直性脊椎炎(AS)及继发性脊椎炎鉴别诊断的可能性及B27 DNA亚型差异对流式B27平均荧光强度的影响;对流式(FCM)B27/B7双标法用于AS患者B27表达情况的测定进行评估。方法采用FCM双标法、微量淋巴细胞毒试验(CDC)及引物特异性多聚酶链反应(SSP-PCR),对76例正常人、57例原发AS病和106例继发性脊椎炎进行B27测定;并用寡核苷酸探针杂交(SSOPH)对50例B27血清学阳性原发AS进行B27 DNA分型。结果原发性AS及继发性脊椎炎患者B27平均荧光强度差异无统计学意义(P>0.05);HLA B27 04型和HLA B27 05型的流式B27平均荧光强度差异无统计学意义(P>0.05);CDC、SSP-PCR和FCM双标法特异性和敏感性分别为76.3%和78.5%、88.4%和86.3%、86.8%和88.85%。结论FCM B27平均荧光强度尚不能用于原发性AS及继发性脊椎炎患者鉴别诊断;不同B27 DNA亚型的B27平均荧光强度可能无差异;FCM HLA-B 27/B7双标法是一种快捷、简便、高效的HLA-B27检测方法。 Objective To evaluation of the probability of flowcytometric measuremen (FCM) as a clinical tool in differentiating primary AS and secondary spondylitis and FCM of HLA-B27 antigen and the effects of B27 DNA subtypes on the mean channel of fluorescence of FCM. Methods We determined the HLA-B27 on lymphocytes of 163 cases of primary AS nad secondary spondylitis and 76 cases of negative control using HLA-B27 monoelonal antibody by FCM. Data were compared to those by CDC and SSP-PCR .50 cases of serological B27 positive was subtyped by SSOPH. Results There was no significant difference between primary AS and secondary spondylitis patients as measured by mean. hamel of fluorescence of FCM, nor was there any different between HLAB 2704 and HLAB 2705. The specificity and sensitivity of three methods were respectivey 76.3 % and 78.5 %, 88.4 %, a nd 86.3 %, 86.8 % and 88.85 %. Conclusion Our data suggested that Mean channel of fluorescence of FCM Can't used for differentiating primary AS and secondary spondylitis and There my not be any difference of Mean channel of fluorescence of FCM between different B27 DNA subtypes and FCM is a rapid and reliable method for HLA-B27 determination.
