摘要
目的:探讨可溶性E-选择素表达对结直肠癌诊断及预后判断的临床意义。方法:采用ELISA法检测结直肠癌(结直肠癌组)307例、结直肠良性疾病(良性疾病组)38例和正常人(正常对照组)30例可溶性E-选择素血清水平,分析其阳性表达与肿瘤临床病理特征的关系,并与CEA、CA199和CA242比较特异性、敏感性。结果:结直肠癌组血清可溶性E-选择素表达水平为(66.82±27.91)ng/mL,与正常对照组[(17.94±5.53)ng/mL]及良性疾病组[(21.65±8.18)ng/mL]比较,差异具有非常显著性统计学意义(P<0.01)。血清可溶性E-选择素阳性表达与肿瘤部位及组织学分型无明显相关(P>0.05),但与Dukes病理分期及肝转移呈显著正相关(P<0.05,P<0.01)。结直肠癌组血清可溶性E-选择素、CEA、CA199和CA242阳性率分别为65.47%、36.16%、26.71%和33.55%,可溶性E-选择素与另3项常规肿瘤标志物阳性率比较,差异具有非常显著性统计学意义(P<0.01),采用3项常规肿瘤标志物平行联检法,其诊断灵敏度为52.20%,明显低于可溶性E-选择素阳性率(P<0.01)。结论:可溶性E-选择素有可能成为结直肠癌早期辅助诊断、预测复发转移及评估预后有价值的肿瘤标志物。
Objective:To study positive rate of soluble E-selection in serum and its clinical significance in colorectal cancer patients. Methods:Serum E-selection was detected with ELISA in 307 cases of colorectal cancer,38 cases of benign colorectal diseases and 30 healthy volunteers. Comparison between E-selection with CEA, CA199 and CA242 was performed with the specificity and the sensitivity in diagnosing colorectal cancer. Results:The serum level in the patients was(66.82 ± 27.91 )ng/mL, and the level was higher than that in the patients with the benign diseases( 21.65 ± 8.18 ) ng/mL and the healthy volunteers( 17.94 ± 5.53 ) ng/mL. The differences were significant( P 〈 0.01 ). Serum E-selection level was correlated with Dukes stage and hepatic metastasis in colorectal cancer,but not with, focal location and histological grade( P 〉 0.05 ). The positive rate of E-selection in hepatic metastasis serum was higher than that in no hepatic metastasis( P 〈 0.01 ). The detection rate of E-selectin, CEA, CA199, and CA242 was 65.47% ,36.16% ,26.71% and 33.55% , respectively. The differences of the detection rates were significant( P 〈 0.01 ). The sensitivity of simultaneous determination with CEA, CA199 ,and CA242 was 52.20%. It was lower than the positive rate of E-selection( P 〈 0.01 ). Conclusion:E-selection may be a valuable marker for early diagnosis and prognosis of colorectal cancer patients.
