摘要
目的探讨临产孕妇全血细胞参数、凝血功能指标的变化及其临床意义。方法采用日本Sysmex K-4500血细胞分析仪、法国STA-R全自动凝血仪,对530例临产孕妇和140例健康妇女进行全血细胞参数、凝血功能指标的检测和分析。结果临产孕妇白细胞计数、粒细胞百分比、红细胞体积分布宽度、血小板计数、血小板平均体积、血小板体积分布宽度、大血小板比率、纤维蛋白原等指标高于健康对照组,两组差异有统计学意义(P<0.05或P<0.01);淋巴细胞百分比、中间细胞百分比、红细胞数、血红蛋白浓度、红细胞压积、红细胞平均血红蛋白量、红细胞平均血红蛋白浓度、凝血酶原时间、凝血酶原时间国际标准比值、活化部分凝血活酶时间低于健康对照组,差异有统计学意义(P<0.01);红细胞平均体积、凝血酶时间与健康对照组比较差异无统计学意义(P>0.05)。结论临产孕妇可出现不同程度的缺铁性贫血,且血液处于高凝状态,容易发生弥漫性血管内凝血。
Objective To investigate the change of complete blood cell parameters and blood coagulation indices in parturient pregnant women and its clinical significance. Methods 530 cases of parturient pregnant women （parturient group） and 140 cases of non-pregnant women （control group） were detected for blood cell parameters and blood coagulation indices with Sysmex K-4500 blood cell analyzer and STA-R automatic blood coagulation analyzer. Results White blood cell count （WBC）, granulocyte percentage （GRA%）, red blood cell volume distribution width （RDW）, platelet count （PLT）, mean platelet volume （MPV）, platelet distribution width （PDW）, platelet large cell ratio （P-LCR%） and fibrinogen （Fib） were significantly higher （P〈0. 05 or P〈0.01）; lymphocyte percentage （LYM%）, mid cell ratio （MID%）, red blood cell count （RBC）, hemoglobin （Hb）, hematocrit （HCT）, mean corpuscular hemoglobin （MCH）, mean corpuscular hemoglobin concentration （MCHC）, prothrombin time （PT）, prothrombin time-international normalized ratio （PT-INR）, and activated partial thromboplasting time （APTT） were significantly lower （P（0.01） in parturient group than those of control group. There were no statistical differences of mean corpuscular volume （MCV） and thrombin time （TT） between parturient group and control group （P〉0. 05）. Conelusion Different degree of iron deficiency anemia and hypercoagulabale state occur in parturient pregnant women, which is liable to disseminated intravascular coagulation （DIC）.
