摘要
目的研究CD147作为生物学标记物预测原发性肝癌(HCC)病人切除术后复发的临床意义。方法应用抗CD147抗体对111例行肝癌切除术的HCC病人的石蜡切片进行免疫组织化学染色;结果分析采用Logrank检验、KaplanMeier分析以及多因素Cox回归分析。结果CD147主要表达在HCC癌组织中,在癌旁间质组织中呈阴性表达;CD147在癌组织中的表达与肿瘤大小(P〈0.0001)、pTNM肿瘤分期(P=0.0001)、门脉癌栓(P=0.0013)以及术前血清AFP水平有显著正相关性;CD147在癌组织中呈阳性表达的HCC病人的无瘤生存率明显低于CD147呈阴性表达的HCC病人的无瘤生存率(P〈0.0001),多因素Cox回归分析显示,CD147在癌组织中的强阳性表达与HCC病人切除术后复发密切相关(P〈0.0001)。结论CD147可作为一种有效的预测HCC切除术后复发的生物学标记物。
Objective To investigate the prognostic role of CD147 as a tumor biological marker in predicting recurrence of hepatocellular carcinoma (HCC) after resection. Methods Immunohisto chemical staining of HCC tissue samples from 111 patients was performed using anti-CD147. The survival curves were estimated by Kaplan-Meier analysis and the prognostic significance of the marker was analyzed using the log-rank test. In addition, the identification of relevant prognostic factors was performed by multivariate Cox regression analysis. Results CD147 was detected mainly in cancerous lesions, but not in stromal cells and endothelial cells adjacent to cancer lesions. CD147 was positively correlated with the presence of tumor size (P〈0. 0001), pTNM tumor stages (P= 0. 0001), venous invasion (P=0. 0013) and serum -fetoprotein (AFP) level (P〈0. 0001). Patients with positive ex pression of CD147 had worse tumor recurrence-free survival than those with negative one (P〈 0. 0001). The multivariate Cox regression analysis showed that strong expression of CD147 was significantly associated with recurrence after resection in patients with HCC (P〈0. 0001). Conclusions The CD147 may be a significant tumor biological predictor of recurrence after resection of HCC.
