氢氧化钙在外伤脱位再植牙牙髓坏死治疗中的应用 被引量:1

摘要 目的观察氢氧化钙作为根管内封药在外伤脱位牙牙髓坏死后根管治疗术中的效果。方法选择牙根已形成脱位,时间超过2h的59颗前牙,随机分为实验组和对照组。实验组再植后1周根管内置氢氧化钙,2周后完成根管充填;对照组再植前行常规根管治疗。对术后牙根吸收及失牙情况进行跟踪观察并分析其原因。结果术后2周拆除固定装置,59颗再植牙均无松动,再植成功率100%。随访2年,对照组中8例出现牙根吸收,牙松动脱落,2年再植成功率72.4%。实验组中2例出现牙根吸收,松动脱落,2年再植成功率93.3%。实验组与对照组2年再植成功率差异有统计学意义(χ2=4.58,P<0.05)。结论外伤性脱位前牙再植后1周去髓,髓腔置氢氧化钙可减少牙根吸收,提高牙再植成功率。 Objective To investigate the effect of calcium hydroxide as intracanal medication in root canal therapy of pulp necrosis of replanted tooth. Method 59 anterior-teeth with dislocation time over 2 hours were randomly divided into experimental group ( one week after replantation, calcium hydroxide was applied in former canal, and the root canal filling was completed after 2 weeks) and control group (routine root canal therapy was conducted before replantation). After operation, the situation of root resorption and missing tooth tracking observation were made and analysed. Result The replantation success rate of 59 teeth reached 100%. Follow up visit for 2 years, the number of cases appeared root resorption and odontoptosis were 8 cases in control group and 2 cases in experimental group while the 2-year replantation success rate were 72.4% in control group and 93.3% in experimen- tal group ( X^2 = 4.58, P 〈 0. 05 ). Conclusion Calcium hydroxide as intracanal medicament can reduce root resorption, and the replantation success rate increased.
