实验性脑梗死后出血性转化大鼠脑损伤机制的研究 被引量:5

摘要 目的建立稳定的实验性脑梗死后出血性转化(hemorrhagic transformation,HT)大鼠模型,对HT后神经功能缺损、脑水肿、脑组织病理学改变及脑组织三磷酸腺苷酶(adenosine triphosphatase,ATPase)、琥珀酸脱氢酶(succinic dehydrogenase,SDH)、丙二醛(malondialdehyde,MDA)进行动态观察,探讨HT后继发脑损伤的机制。方法健康雄性Sprague-Dawley大鼠随机分为出血性转化组(HT组)、脑梗死组(CI组)和假手术组(Sham组)。应用两步法建立HT大鼠模型。术后3、6、12、24h进行神经功能评分,脑组织含水量,脑组织ATPase、SDH和MDA水平的测定。采用2,3,5-三苯基四唑氮红(triphenyltetrazolium chloride,TTC)染色大体观察及采用光学显微镜、透射电子显微镜进行脑组织病理学观察。结果术后各时间点HT组和CI组大鼠神经功能评分、脑组织含水量间差异均无统计学意义(P>0.05)。术后12h、24h HT组脑组织ATPase活性较CI组差异有统计学意义(P<0.01);术后24h HT组脑组织SDH活性较CI组差异有统计学意义(P<0.05);术后12h HT组脑组织MDA水平较CI组差异有统计学意义(P<0.01)。HT组脑组织经TTC染色后在白色梗死区中可见一类圆形血肿。病理学观察可见神经元退变、间质水肿、超微结构线粒体肿胀、内质网扩张等。结论CI继发中等量出血的HT与单纯CI比较,神经功能缺损、脑水肿及脑损伤程度均无明显恶化,提示CI患者继发HT后临床症状可能并无明显改变。 Objective To establish a stable hemorrhagic transformation (HT) model in rats so as to study the neurobehavioral deficit, brain edema, pathological changes,brain adenosine triphosphatase (ATPase) activity, succinic dehydrogenase (SDH) activity and malondialdehyde (MDA) content,in order to explore the mechanism of brain injury after HT.Methods Healthy male Sprague-Dawley rats were divided randomly into HT group,cerebral infarction (CI) group and Sham group.Two steps were used to develop the HT model.Neuronal behavior was evaluated at 3rd,6th,12th and 24th hours after operation.And then rats were sacrificed for measurement of brain water content, brain ATPase activity, SDH activity and MDA content.Triphenyltetrazolium chloride (TTC) staining was used to observe the brain macro-pathologically,with light microscope and transmission electron microscope.Results There were no significant differences in neuronal function score and brain water content between the HT group and CI group at various time points (P〉0.05).There was significant difference in brain ATPase activity between the HT group and CI group at 12 h and 24 h (P〉0.01); there was significant difference in brain SDH activity between the HT group and CI group at 24 h (P〉0.05),and there was significant difference in brain MDA content between the HT group and CI group at 12 h (P〉0.01).Brain samples of HT group stained in TTC at 24 h after operation showed that in the white infracted area,there were a circular hematoma,neuronal degeneration and interstitial edema pathologically,and mitochondrial swelling and endoplasmic reticular dilation in ultromicrostrure.Conclusion A moderate HT after CI does not aggravate in neurobehavioral deficit,brain edema and brain injury severity reflected by brain ATPase activity,SDH activity and MDA content,which suggests that the clinical symptoms of the CI patient complicated by HT may have no obvious change.
