COPD低氧及肺性脑病患者血清血管紧张素Ⅱ检测的临床意义 被引量:1

The clinical significance of serum angiotensin Ⅱ in hypoxic with COPD and pulmonary encephalopathy

摘要 目的探讨慢性阻塞性肺病(COPD)低氧及肺性脑病患者血清中AngⅡ检测的临床意义。方法住院COPD患者45例,采取全自动血气分析仪测定PH值、氧分压(PaO2)、二氧化碳分压(PaCO2),根据临床症状及血气分析将其分为单纯低氧组30例、肺性脑病组15例,用放射免疫法测定两组血清中AngⅡ含量,选择正常对照组15例相比较。结果与单纯低氧组和正常对照组相比,肺性脑病组的PaCO2明显升高(P<0.01),PH及PaO2明显降低(P<0.01);单纯低氧组和肺性脑病组AngⅡ明显高于正常对照组(P<0.01),肺性脑病组AngⅡ明显高于单纯低氧组(P<0.01),肺性脑病组PaO2与AngⅡ呈直线负相关(r=-0.667,P<0.01),PaCO2与AngⅡ呈直线正相关(r=0.651,P<0.01)。结论血清中AngⅡ的检测对肺性脑病的诊治有一定的帮助。 Objective To investigation the changes of the serum level of angiotensin Ⅱ ( Ang Ⅱ) in hypoxic with chronic obstructive pulmonary disease (COPD) and pulmonary encephalopathy and their clinical significance. Methods 45 patients with COPD from hospitalization were divided into two groups Measures included adopted automatic blood gas analyzer of pH, oxygen tension ( PaO2 ), carbon dioxide partial pressure ( PaCO2 ), the basis of clinical symptoms and blood gas analysis the immune serum angiotensin. 30 cases were in the hypoxic group alone and 15 in the group with with radiation. The normal group had 15 cases for comparison. Results. In the hypoxia alone group, the normal control group, and the pulmonary encephalopathy group, PaCO2 increased significantly (P 〈0. 01 ), PH and PaO2 decreased significantly (P 〈 0. 01 ). The serum level of angiotensin Ⅱ (Ang Ⅱ) in the hypoxia alone group and pulmonary encephalopathy group was significantly higher than that of the control group ( P 〈0. 01 ), pulmonary encephalopathy group was significantly higher in angiotensin Ⅱ than that in the hypoxia alone group ( P 〈0. 01 ). The serum level of angiotensin Ⅱ was negatively correlated with PaO2 in the pulmonary encephalopathy group ( r = - 0. 667, P 〈 0. 01 ) and was positively correlated with PaCO2 ( r = 0. 651, P 〈 0. 01 ). Conclusion Elevated serum angiotensin Ⅱ, through the promotion of pulmonary hypertension or directly involved in the formation of brain edema, increased pulmonary encephalopathy. The measure of serum angiotensin Ⅱ can help detect pulmonary encephalopathy in the diagnosis and the treatment.
