摘要
目的:研究直肠癌组织及其远侧端黏膜组织中CD34表达并从分子病理学水平上探讨直肠癌的安全远侧切缘。方法:对45例中低位直肠腺癌下缘进行标记;PET/CT检查并结合半定量免疫组织化学方法对直肠癌及其远侧端黏膜CD34所表达的微血管密度进行研究。结果直肠腺癌肿瘤组织中微血管密度明显高于直肠腺癌远侧端的直肠黏膜组织,直肠腺癌远侧端黏膜微血管密度随着距离增大而减少,但与阴性对照有显著性异常改变只在有限的长度内(直肠远侧端1.0cm)。结论:微血管密度的检测为直肠癌抗血管治疗提供理论基础;直肠癌分子病理学的安全远侧切缘应大于1cm。
Objective:To study the expression of CD34 in rectal cancer and its distal mucosa and to explore the required distal margin of radical surgery for rectal cancer in molecular pathologic level. Methods: All 45 cases of the rectal cancer were marked before operation, and detected by PET/CT imaging; CD34 expression in rectal cancer and rectal distal tissue portion were detected by using immunohistochemical technique. Results: MVD (microvessel density) of CD34 expression in rectal cancer was significantly higher than that in distal mucosa to cancer margin,which was decreased in distal mucosa along the anal direction. But it was different between only in less than 1 cm distal rectal mucosa and in negative control rectal tissue. Conclusion : The detection of MVD in the rectal cancer provided the basis of antiangiogenesis treatment of rectal cancer, The required distal margin of radical surgery for rectal cancer in molecular pathologic level should be longer than 1.0cm.
