摘要
目的探讨长期细胞清洗透析是否较普通透析更具优势。方法将21例维持性血液透析患者随机分成OD组(接受普通透析ordinary dialysis,OD)和CWD组(接受细胞清洗透析cell-wash hemodialysis CWD),两组均应用Fresenius4008B透析机维持血液透析1年;OD组使用钠浓度恒定的透析液,CWD组交替使用钠浓度为150mmol/L和130mmol/L的透析液。检测两组平均动脉压(MAP)、干体重(DBW)、血肌酐(SCr)和血红蛋白(Hb),并计算尿素氮反跳率(RRBUN)和二室尿素清除指数(Kt/Vdp)。结果①与OD组比较,CWD组MAP下降更明显(4.4±1.5VS10.6±3.4mmHg),DBW明显增加(0.8±0.3VS2.3±0.9Kg),贫血改善(95.4±17.6VS125.5±10.0g/L),RRBUN降低(4.7±1.5%VS2.1±0.6%),Kt/Vdp增加(1.4±0.4VS1.8±0.3);②CWD组肌痉挛和低血压发生率均明显低于OD组。结论长期CWD可提高Kt/Vdp,能更彻底清除细胞内外毒素、改善患者一般状态、增加透析耐受性。
Objective To compare long-term cell-wash hemodialysis with ordinary hemodialysis. Methods 21 maintenance hemodialysis patients were randomly divided into two groups: group OD(accepted ordinary hemodialysis) and group CWD(accepted cell-wash hemodialysis). Mean arterial pressure (MAP), dry body weight (DBW), serum creanine (Scr) and hemoglobin (Hb) were mesured , ureanitrogen rebound rate (RRBUN)and Kt/Vdp were calculated. Results Compare to group OD, patients in group CWD had lower MAP and RRBUN and distinct high DBW, Hb and Kt/ Vdp, and a lower incidence of muscle spasm and low blood pressure. Conclusions Long-term cellwash hemodialysis may remove more uremia toxin, enhance tolerance to hemodialysis.
