摘要
目的对引起感染的酵母样真菌类型及其耐药特性进行研究,以指导临床合理用药。方法从2005年1月~2007年12月我院临床送检的血液、中段尿、粪便、痰、胸腹水、伤口分泌物、穿刺液等标本中,采用常规方法培养和分离酵母样真菌,用法国生物梅里埃ATB-EXPREss系统对酵母样真菌进行鉴定,用ATB-FUNGUS药敏试条进行药敏试验。结果共分离到484株酵母样真菌,其中白色假丝酵母菌占42.4%,热带假丝酵母菌占27.9%,光滑假丝酵母菌占10.5%,清酒假丝酵母菌占7.0%,近平滑假丝酵母菌占2.3%,克柔假丝酵母菌占2.7%,中间假丝酵母菌占4.1%,其他酵母菌占3.1%。酵母样真菌对两性霉素B敏感,对5-氟胞嘧啶、氟康唑、伊曲康唑存在不同程度的耐药。结论临床需要加强真菌送检培养,合理使用抗真菌药物,尽量避免单纯经验用药,以减少耐药菌株的出现。
Objective To investigate the species of pathogenic yeast like-fungi and their drug resistance in order to guide rational clincal administration. Methods Samples including blood, urine, feces, sputum, pleural effusion,ascites, wound secretion and puncture fluid sent for fungus detection in this hospital, from Jan. 2005 to Dec. 2007, were cultured by routine methods, and identified with France Biomerieux TB-express system. A drug sensitivity test was done by ATB-fungus strip. Results A total of 484 strains were isolated, in which, candida albicans (42. 4%), C. tropicalis (27. 9%), C. glabrata (10. 5%), C. sake (7.0%), C. parapsilosis (2.3%), C. krudei (2.7%), C. intermedia (4.1%) and others (3.1%) were noted. All these were sen sitive to amphotericin B, and expressed resistance to 5-fluorocytosine, fluconazole and itraconazole in different extent. Conclusion Yeast like-fungi are required to be sent for classification and drug resistance study so as to rationally use anti fungal agent to reduce the development of drug-resistant strains.
