摘要
①目的研究体外循环中脂质过氧化对红细胞钙的影响。②方法对行体外循环的病儿分别于转流前、转流3min,转流结束、转流后2h,24h采抗凝血。测定血浆及红细胞膜过氧化脂质、红细胞钙、红细胞膜Ca2+,Mg2+-ATP酶和Na+,K+-ATP酶活性。③结果转流3min时血浆和红细胞膜过氧化脂质即增高,转流结束达到高峰(t=3.527,2.921,P均<0.01);转流开始红细胞钙即升高(t=2.937,P<0.01),转流结束后2h达到高峰(t=3.426,P<0.01);红细胞膜Ca2+,Mg2+-ATP酶和Na+,K+-ATP酶活性转流3min时暂时性升高后迅速降低,转流结束后2h达最低点(t=2.816,2.852,P均<0.01)。转流结束后24h除红细胞钙仍高于转流前(t=2.028,P<0.05)外,其它各值均恢复正常。④结论氧自由基介导的脂质过氧化损伤是体外循环中红细胞钙超负荷的重要原因之一。
Objective To study the relationship between lipid peroxidation and erythrocyte calcium during cadiopulmonary bypass(CPB), and discuss the mechanism of erythrocyte calcium overload. Methods Twentyfour patients were performed open heart surgery.Blood samples were obtained from radial artery before CPB, 3 minutes after CPB, on termination of CPB, 2 hours and 24 hours after the end of CPB respectively. We measured the lipid peroxidation (PLPO, EmLPO), erythrocyte calcium, Na+,K+ATPase and Ca+2,Mg2+ATPase. Results PLPO, EmLPO and calcium of erythrocyte increased after 3 minutes CPB. PLPO and EmLPO reached the highest level at the end of CPB(t=3.527,2.921,P<0.01). Erythrocytes calcium reached the highest level 2 hours after CPB(t=3.426,P<0.01). Na+, K+ATPase and Ca2+,Mg2+ATPase elevated 3 minutes after the beginning of CPB, then sharply decreased and dropped to the lowest level 2 hours after CPB(t=2.816,2.852,P<0.01). All the items dropped to the normal level 24 hours after CPB except ECa2+ which still at high level(t=2.082,P<0.05). ConclusionFree radicals induced lipid peroxidation was one of the important factors in causing erythricyte calcium overload during CPB.
