摘要
目的应用超声多普勒探讨在体肱动脉剪应力对肱动脉内皮的功能与血管结构的影响。方法分别对正常人及糖尿病患者各40例,采用高频超声测算肱动脉最大壁面剪应力(τp)及平均壁面剪应力(τm),测量肱动脉内中膜(IMT),同时测定血管反应性充血扩张度(FMD%),分析τm与IMT、内皮功能受损的相关关系。结果内皮功能受损、肱动脉IMT增厚的糖尿病组与正常对照组相比,肱动脉τp与及τm均显著减小(P<0.01)。τp、τm与IMT呈明显的负相关(r值分别为-0.76与-0.58,P均<0.001),τp、τm与FMD%呈明显正相关(r值分别为0.73与0.64,P值均<0.001)。结论本研究提示了τm减低对于肱动脉血管内皮结构及内皮功能改变的慢性作用。
Objective To analyze the effect of wall shear stress of brachial artery (BA) on endothelial function and intima-media thickness (IMT) of brachial artery (BA). Methods Forty patients with diabete mellitus (DM) were selected and 40 healthy subjects as control. Ultrasound images of BA were collected and IMT of BA was measured in vivo. The peak wall shear stess(τp) and the mean wall shear stess(τm) of BA and flow-mediated dilation(FMD% )were determined by B-mode ultrasound and pulsed-Doppler. Results Compared with control group,τp and τm of BA were significantly decreased in DM group. Its IMT of BA was thicker and endothelial function was injured. Significant negative correlation of τp or τm with IMT ( r= - 0.76 and -0.58 ,P 〈0.001 ) and positive correlation of τp or τm with FMD% ( r =0.73 and 0.64,P 〈0. 001 ) existed respectively. Condusion The chronic effect of wall shear stress on endothelial structure and function of BA was verified.
