摘要
观察酒石酸布托啡诺在剖宫产手术中行硬膜外麻醉的效果。方法90例择期行剖宫产手术患者随机分为两组,观察组(酒石酸布托啡诺组)和对照组(芬太尼组),两组均采用硬膜外麻醉方式,观察术前和麻醉后15 min的血压、心率、血氧饱和度,术中镇静评分,牵拉腹膜、缝合子宫时的牵拉反应,新生儿Apger评分,术后6,12,24 h的VAS评分以及术后首次排气时间。结果观察组麻醉后15 min的血压、心率与术前比较有显著性差异(P<0.05);术中镇静评分观察组明显高于对照组(P<0.05);术中牵拉反应,在牵拉腹膜、缝合子宫等腹腔内主要操作步骤时,牵拉反应为Ⅰ、Ⅱ级的病人数观察组明显低于对照组(P<0.05)。结论酒石酸布托啡诺镇痛作用可靠,麻醉效果确切,不良反应发生率较低,为剖宫产手术理想的麻醉选择。
Objective To observe the effect of pidural anesthesia with butorphanol tartrate in caesarean section. Methods 90 cases undergoing elective caesarean section were randomly divided into 2 groups, the observation group（ butorphanol tartrate） and the control group （fentanyl）, both of which employed epidural anesthesia. The blood pressure, heart rate and satration of blood oxygen before operation and 15 min after anesthesia, sedation score in operation, drag reaction when dragging peritoneum and suturing uterus, Apger score of neonates, VAS score of postoperative 6h, 12h and 24h, and initial air bleeding time after operation were recorded. Resuits In the observation group, there was signification difference （ P 〈 0.05 ） in the blood pres- sure, heart rate and satration of blood oxygen between 15 min after anesthesia and before operation. The sedation score in operation in the observation group was significantly superior to that in the control group （P 〈0.05）. The cases of drag reaction of grade I and II in the observation group was significantly inferior to that in the control group （P 〈 0.05）. Conclusion Butorphanol tartrate, with reliable analgesic effect, definite anesthetic effect and low incidence rate of adverse reaction, can be selected as an ideal anesthesia medicine in caesarean section.
